Pharmacometabonomic association of cyclophosphamide 4-hydroxylation in hematopoietic cell transplant recipients

Summary: This study used global metabolomics to identify endogenous metabolomic compounds associated with the clearance of 4-hydroxycyclophosphamide (4HCY), a cytotoxic metabolite of cyclophosphamide (CY). The results showed that certain metabolites were associated with 4HCY clearance at different time points, suggesting the potential of pharmacometabolomics in optimizing the use of CY.

The results showed the feasibility of lipidomics but further studies are needed to optimize CY dosing in larger samples.

Summary: Nonadherence to the medical regimen after allogeneic hematopoietic cell transplantation (HCT) is a significant issue, but little research has been conducted on this topic. This study aimed to determine the rates and predictors of nonadherence during the first 8 weeks after hospital discharge. The findings revealed variations in nonadherence rates among different tasks, and both patient and caregiver factors, particularly self-efficacy, were important predictors of nonadherence.
