The complex mechanisms underlying obesity-related cardiovascular disease pose challenges for clinical practice. Intentional weight loss can reduce cardiovascular risk, but long-term maintenance requires ongoing commitment. This position paper aims to improve understanding of the links between obesity and cardiovascular disease, address controversies, and provide recommendations for clinicians.

Summary: Metabolically healthy obesity (MHO) was associated with a high risk of cardiovascular disease (CVD) among adults in rural Xinjiang, with metabolically unhealthy obesity (MUO) group having an even higher risk. There were also sex differences in the relationship between BMI-metabolic status and incident CVD. In each BMI category, metabolically unhealthy subjects had a higher risk of developing CVD than did metabolically healthy subjects.

An Empirically Derived Definition of Metabolically Healthy Obesity Based on Risk of Cardiovascular and Total Mortality

Summary: The study derived a new definition of metabolic health (MH) using data from NHANES-III and UK Biobank cohorts, and found that metabolically healthy obesity is not associated with increased risk of cardiovascular disease (CVD) mortality and total mortality compared to metabolically healthy normal weight individuals. The new MH definition may help identify a subgroup of obese individuals without increased mortality risk and stratify risk in overweight or normal weight individuals.

Summary: The study found that individuals with metabolically healthy obesity (MHO) had higher rates of heart failure and respiratory diseases, but not necessarily atherosclerotic cardiovascular diseases. Compared to metabolically healthy individuals without obesity, those with MHO had higher risks of diabetes, cardiovascular diseases, heart failure, respiratory diseases, and all-cause mortality.
