Perioperative changes of FGF23 in patients undergoing surgery for primary hyperparathyroidism

Summary: This study investigated the changes in FGF23 concentrations in pHPT patients during the perioperative period. The results showed that intraoperative FGF23 levels were at the upper reference limit, while postoperative cFGF23 and iFGF23 significantly decreased and were associated with an increase in inorganic phosphate. Additionally, FGF23 levels just before incision correlated with eGFR.

Intraoperative PTH monitoring may improve surgical outcomes, and short-term outcomes after surgery are similar to those of typical primary hyperparathyroidism (TPHPT).

Summary: This study assessed the clinical usefulness of assaying FGF-23, Klotho, osteocalcin, NTX, and sclerostin levels in PHPT patients as bone damage markers and indicators of surgical treatment success. Results showed differences in bone markers between PHPT patients and controls, with normalization of markers post-surgery indicating treatment success. Further research is needed to determine the utility of FGF-23 and Klotho in monitoring PHPT.
