Impact of severe secondary tricuspid regurgitation on rest and exercise hemodynamics of patients with heart failure and a preserved left ventricular ejection fraction

Summary: This study aimed to explore the impact of secondary tricuspid regurgitation (STR) on exercise hemodynamics and cardiorespiratory adaptation in heart failure with preserved ejection fraction (HFpEF) patients. The results showed that HFpEF patients with severe STR had higher right atrial pressure and pulmonary artery wedge pressure, and lower left ventricular transmural pressure and cardiac output compared to HFpEF patients without STR. This impairment in SV and CO reserve in HFpEF-STR led to pulmonary vascular de-recruitment and relative left heart underfilling.

Further study is required to identify innovative treatments for the pulmonary vasculature in PH-LHD.

Exercise haemodynamics in heart failure with preserved ejection fraction: a systematic review and meta-analysis

Summary: This study aimed to analyze the exercise hemodynamics profile of HFpEF cohorts reported in literature and compare them with control subjects. Despite methodological and heterogeneity of the pooled estimates, the exercise hemodynamic profile of HFpEF patients consistently showed a steep rise in PAWP during exercise. There is a need for further standardization of exercise hemodynamics for wider application in clinical practice.

Impact of COVID-19 on exercise pathophysiology: a combined cardiopulmonary and echocardiographic exercise study

Summary: At the time of hospital discharge, patients with COVID-19 present with reduced functional capacity and exercise hyperventilation. Peripheral factors, namely reduced oxygen extraction (myopathy) and anemia, which are not fully compensated by a supernormal cardiac output response, account for exercise limitation before exhaustion of the respiratory reserve. Enhanced chemoreflex sensitivity, rather increased dead space, mainly accounts for exercise hyperventilation. The pulmonary vascular response to exercise circulation of survived patients with COVID-19 does not present major pathological changes.
