Severe hyponatraemia in two patients with breast cancer caused by low-dose cyclophosphamide and precipitated by aprepitant

Summary: This study reports two cases of postmenopausal women with breast cancer who developed acute confusion and seizures after the first cycle of neoadjuvant chemotherapy. They were found to have severe hyponatraemia, with the most likely diagnosis being cyclophosphamide-associated syndrome of inappropriate antidiuretic hormone secretion. The commonly used antiemetic aprepitant was identified as the precipitating factor.
