Vitreous humor proteome: unraveling the molecular mechanisms underlying proliferative and neovascular vitreoretinal diseases

Fatima Milhano dos Santos et al.

Summary: Proliferative diabetic retinopathy (PDR), proliferative vitreoretinopathy (PVR), and neovascular age-related macular degeneration (nAMD) are leading causes of blindness. Proteomics studies have revealed the complexity and biological activity of the vitreous, which plays a central role in the onset and progression of vitreoretinal diseases. Understanding the vitreous proteome could provide insights into the pathological mechanisms underlying these diseases and identify potential pharmaceutical targets.

The Complement System in Retinal Detachment with Choroidal Detachment

Shasha Luo et al.

Summary: This study aimed to investigate the differences in complement components and complement regulatory factors in the vitreous humor of patients with RRDCD and RRD. The results showed that the levels of complement components and complement regulatory factors were significantly increased in the RRDCD group compared to the RRD and control groups. Additionally, the vitreous humor of the RRD group also exhibited increased levels of complement component C2 and CFD. These findings suggest a potential pathophysiologic impact of complement activation in RRDCD and RRD.

Efficacy, durability, and safety of intravitreal faricimab up to every 16 weeks for neovascular age-related macular degeneration (TENAYA and LUCERNE): two randomised, double-masked, phase 3, non-inferiority trials

Jeffrey S. Heier et al.

Summary: The primary results of two Phase 3 trials show that faricimab can effectively improve visual acuity for patients with nAMD at intervals of up to 16 weeks, with comparable safety profile to aflibercept. This suggests that faricimab has the potential to reduce treatment burden by extending the time between injections while maintaining efficacy.

VEGF-targeting drugs for the treatment of retinal neovascularization in diabetic retinopathy

Alessandro Arrigo et al.

Summary: Diabetic retinopathy is a common complication of diabetes that can cause visual impairment. Anti-VEGF therapies have significantly improved the management of proliferative diabetic retinopathy. The role of anti-VEGF as a substitute for laser treatment or as a combination therapy is still debated. Overall, anti-VEGF treatments are considered the first-line choice for diabetic retinopathy management.

Quantitative proteomics analysis of human vitreous in rhegmatogenous retinal detachment associated with choroidal detachment by data-independent acquisition mass spectrometry

Shasha Luo et al.

Summary: This study focused on identifying the proteins and signal pathways associated with the etiology of rhegmatogenous retinal detachment (RRD) with choroidal detachment (RRDCD) and providing guidance for diagnosis and treatment. Differentially expressed proteins (DEPs) were identified using mass spectrometry and bioinformatics analysis. The results indicated that inflammation and immunity, protein degradation and absorption, cell adhesion molecules (CAMs), the hedgehog signaling pathway, and lipid metabolism were functionally related to RRDCD. This study provides important clues and the theoretical basis for understanding the pathogenesis, progression, and prognosis of ocular fundus disease.

Vitreous Humor Proteome: Targeting Oxidative Stress, Inflammation, and Neurodegeneration in Vitreoretinal Diseases

Fatima Milhano Santos et al.

Summary: Oxidative stress refers to the imbalance between pro-oxidants and antioxidants, leading to various retinal disorders. While the human body has defense mechanisms to counteract autoxidation, it may not always be sufficient to prevent cellular damage. Modern lifestyles and environmental factors contribute to increased chemical exposure and stress induction, further exacerbating oxidative stress.

Brolucizumab: A Newly Developed Anti-VEGF Molecule for the Treatment of Neovascular Age-Related Macular Degeneration

Ramin Tadayoni et al.

Summary: This article reviews the unique molecular design of brolucizumab, a single-chain antibody fragment specifically designed for intraocular use, which allows for a higher concentration of material per intravitreal injection compared to other anti-VEGF agents. Clinical trials have shown the efficacy of brolucizumab in treating nAMD, with signals of a more durable treatment effect and the ability to be administered every 3 months to maintain disease control. The preclinical and clinical data provide evidence of sustained disease control with longer injection intervals, potentially reducing the treatment burden in nAMD patients.

Molecular pathogenesis of rhegmatogenous retinal detachment

Tiina Ohman et al.

Summary: System pathology analysis of vitreous proteins in patients with rhegmatogenous retinal detachment revealed specific proteome profiles compared to other eye diseases, indicating activation of mechanisms like glycolysis, photoreceptor death, and signaling pathways to promote retinal cell survival. Additionally, platelet-mediated wound healing processes, reorganization of cell adhesion molecules, and apoptotic processes were also detected during disease progression.
