4D-flow MRI derived wall shear stress for the risk stratification of bicuspid aortic valve aortopathy: A systematic review

Jiaxing Jason Qin et al.

Summary: The current intervention guidelines for BAV-associated AAo dilatation are not accurate predictors of clinical outcomes. This review examines the relationship between WSS and aortopathy in the BAV population, aiming to identify better biomarkers for risk stratification. The review includes 26 studies that demonstrate the elevated WSS magnitude and circumferential WSS in BAV patients, highlighting the potential of WSS as a valid biomarker for BAV aortopathy.

Wall Shear Stress Predicts Aortic Dilation in Patients With Bicuspid Aortic Valve

Andrea Guala et al.

Summary: This study assessed the predictive value of wall shear stress (WSS) for the growth rate (GR) of the ascending aorta (AAo) in patients with bicuspid aortic valve (BAV). The results showed a positive correlation between WSS and its circumferential component with AAo GR. Therefore, the evaluation of WSS may be helpful in the follow-up of these patients.

Summary: This study evaluated the fluid dynamics in the aortic valve and proximal aorta during LVAD support and found that LVAD implantation significantly reduced wall shear stress, which may contribute to LVAD-induced aortic insufficiency and root thrombus.

Association of Regional Wall Shear Stress and Progressive Ascending Aorta Dilation in Bicuspid Aortic Valve

Gilles Soulat et al.

Summary: This study evaluated the predictive role of wall shear stress (WSS) in the growth of the ascending aorta (AAo). It found that higher rates of AAo dilation were associated with larger areas of elevated WSS in patients with bicuspid aortic valve (BAV), suggesting that measuring WSS can help identify BAV patients who require closer follow-up.

Wall Shear Stress Predicts Media Degeneration and Biomechanical Changes in Thoracic Aorta

Miika Kiema et al.

Summary: The study found that wall shear stress (WSS) is associated with media degeneration, elastin content, and aortic wall strength in patients with thoracic aortic aneurysm (TAA). The outer curves of the aortic wall were thinner, more rigid, and tolerated lower failure strains. Higher smooth muscle cell (SMC) density was correlated with lower strength of the aortic wall structure, along with more macrophages detected in patients with severe media degeneration.

Summary: The study found that impairment of elastic properties of the ascending aorta in BAV patients, as assessed by 2D longitudinal strain, is an effective predictor of aortic complications.
