Summary: The goal of parathyroid imaging is to identify the sources of excessive parathyroid hormone secretion before surgery. Various imaging approaches, including ultrasound, 4DCT, 4DMRI, and scintigraphy with 99mTc-Sestamibi, have been used for this purpose. Combining structural imaging with molecular imaging in hybrid approaches, such as PET/CT and PET/MRI, can provide robust information on both structure and function.

Summary: This study found that the use of ioPTH measurement can reduce the risk of conversion and persistent hypercalcaemia in surgery for primary hyperparathyroidism. Additionally, the use of two or three localization procedures can also decrease the risk of persistent hypercalcaemia. In patients with a concordant single lesion, the risk of persistent hypercalcaemia is low.
