Clinically relevant factors associated with a binary outcome of diabetic macular ischaemia: an OCTA study

Da Wei Yang et al.

Summary: This study investigated the factors associated with diabetic macular ischaemia (DMI) as assessed by optical coherence tomography angiography (OCTA). The results showed that age, visual acuity, ganglion cell-inner plexiform layer thickness, diabetic retinopathy severity, haemoglobin A1c level, estimated glomerular filtration rate, and low-density lipoprotein cholesterol level were associated with SCP-DMI. In addition, the presence of diabetic macular oedema and shorter axial length were associated with DCP-DMI.

Diabetic macular ischemia

Beatrice Tombolini et al.

Summary: This review examines a multimodal imaging approach, including optical coherence tomography angiography (OCTA), for detecting diabetic macular ischemia (DMI). OCTA has emerged as a promising and reliable procedure for qualitatively and quantitatively describing DMI. It can also identify preclinical alterations and predict disease progression before the onset of diabetic retinopathy.

Capillary density and caliber as assessed by optical coherence tomography angiography may be significant predictors of diabetic retinopathy severity

Sam Kushner-Lenhoff et al.

Summary: This study validates the associations between retinal capillary density and caliber with the severity of diabetic retinopathy (DR) in different clinical settings. The addition of vessel skeleton density (VSD) and vessel diameter index (VDI) significantly improves the discriminatory performance of the model, allowing for rapid assessment of DR severity.

Diabetic macular ischaemia- a new therapeutic target?

Chui Ming Gemmy Cheung et al.

Summary: This review discusses the definition and grading issues of diabetic macular ischaemia (DMI), proposing vascular phenotypes of DMI based on relative perfusion deficits. The review highlights mechanistic pathways of DMI, including the role of hypoxia and the complex relation between neurons, glia, and microvasculature. Utilising optical coherence tomography angiography (OCTA) to identify reversible DMI phenotypes and developing new trial endpoints for disease progression are crucial. Natural history studies of DMI are needed to define phenotypes and understand the development of diabetic macular oedema.

Living Without a Diagnosis: A Patient's Perspective on Diabetic Macular Ischemia

Jacqueline D. Humphreys et al.

Summary: Diabetic macular ischemia is a common complication of diabetic retinopathy that can result in vision loss. Despite the lack of consensus and information on prognosis, prevalence, and diagnostic criteria, it is crucial to provide sufficient, accessible, and accurate information to support patients, caregivers, and physicians. Developing confirmed diagnostic criteria for DMI would assist patients in accessing validated information and support the development of treatments. Trust and open communication between patients and physicians are important in managing health anxiety and ensuring adherence to beneficial treatments.

Peripapillary Microvasculature Predicts the Incidence and Development of Diabetic Retinopathy: An SS-OCTA Study

Meng Yuan et al.

Summary: Peripapillary microvascular metrics measured by swept-source optical coherence tomography angiography (SS-OCTA) are associated with the incidence and development of diabetic retinopathy (DR) in a 2-year follow-up study. Decreased peripapillary vascular density (pVD) and peripapillary vascular length density (pVLD) are significantly associated with an increased risk of DR incidence. However, these metrics are not significantly associated with DR progression. The use of these metrics improves the prediction of DR incidence and development.

Looking Ahead: Visual and Anatomical Endpoints in Future Trials of Diabetic Macular Ischemia

Chui Ming Gemmy Cheung et al.

Summary: The review discusses the current clinical challenges of diabetic macular ischemia (DMI) and emphasizes the need for validated endpoints for clinical trials and further research. Potential endpoint measures are identified for monitoring changes in DMI progression, with recommendations for studying the natural history and progression of the condition. This comprehensive approach aims to improve understanding, treatment, and consensus in DMI management.

Diabetic Macular Ischemia: Influence of Optical Coherence Tomography Angiography Parameters on Changes in Functional Outcomes Over One Year

Andrew S. H. Tsai et al.

Summary: This study found that changes in parafoveal microvasculature over one year in diabetic patients can be detected using OCTA. A larger baseline foveal avascular zone area in the deep vascular plexus predicted worsening visual outcomes, while larger decreases in superficial vascular plexus vessel density were associated with worsening retinal sensitivity over the course of one year.
