Meta-analysis of synovial fluid polymerase chain reaction for diagnosing periprosthetic hip and knee infection

Cheng Li et al.

Summary: This study aimed to assess the diagnostic performance of synovial fluid polymerase chain reaction (PCR) in periprosthetic hip and knee infection and determine its diagnostic significance compared to conventional methods. The results showed that synovial fluid PCR has a high sensitivity and specificity in the rapid diagnosis of periprosthetic joint infection and early-stage culture-negative bacterial infections.

Clinical Outcomes of Culture-Negative and Culture-Positive Periprosthetic Joint Infection: Similar Success Rate, Different Incidence of Complications

Zhiyang Xu et al.

Summary: Comparing the clinical outcomes of culture-negative periprosthetic joint infection (CN PJI) with those of culture-positive periprosthetic joint infection (CP PJI), it was found that the treatment success rate was similar between the two groups, but the CN PJI group may have a higher incidence of antibiotic-related complications.

Against the Norm: Do Not Rely on Serum C-Reactive Protein and White Blood Cell Count Only When Assessing Eradication of Periprosthetic Joint Infection

Farouk Khury et al.

Summary: The delta change in serum CRP and WBC before and after two-stage revision arthroplasty for PJI was not associated with reinfection risk. Although inflammatory serologies showed a decreasing trend before reimplantation, the role of CRP and WBC in determining the optimal timing of reimplantation seemed dispensable. Planning for a second-stage reimplantation should involve assessing multiple variables rather than relying on specific numeric changes in these inflammatory marker values.

Symptom duration is associated with failure of periprosthetic joint infection treated with debridement, antibiotics and implant retention

Hongyi Shao et al.

Summary: This study investigates the use of debridement, antibiotics, and implant retention (DAIR) as a treatment strategy for periprosthetic joint infection (PJI). The overall treatment success rate was found to be 67.3% in the cohort of 104 patients. Patients with infectious symptoms lasting longer than ten days had a higher likelihood of treatment failure. There was no difference in failure rate among acute, hematogenous, and chronic infections.

Higher 1-year risk of implant removal for culture-positive than forculture-negative DAIR patients following 359 primary hip or knee arthroplasties

Joyce van Eck et al.

Summary: This study examined the culture results after Debridement, Antibiotics, and Implant Retention (DAIR) in patients with early prosthetic joint infection (PJI) and compared the outcomes between culture-positive and culture-negative groups. The results showed that the relative risk of prosthesis removal was significantly higher in the culture-positive group, which also had higher body mass index, longer duration of wound leakage, and required more subsequent treatments.

Periprosthetic knee infection: treatment options

Michele Vasso et al.

Summary: Infection is a common and severe complication following total knee arthroplasty, which can result in the failure of the procedure. The treatment options for periprosthetic joint infection are still debated and depend on various factors, including implant integrity, infection timing, host factors, infecting organism virulence, and patient preferences.

Outcome of Debridement, Antibiotics, and Implant Retention With Modular Component Exchange in Acute Culture-Negative Periprosthetic Joint Infections

Venkatsaiakhil Tirumala et al.

Summary: The study evaluated the outcomes of DAIR with modular component exchange in acute culture-positive and culture-negative PJI, finding that culture negativity may not be a contraindication to DAIR in patients with acute PJI as the reinfection rates were similar to culture-positive PJI.

One-stage revision is as effective as two-stage revision for chronic culture-negative periprosthetic joint infection after total hip and knee arthroplasty A RETROSPECTIVE COHORT STUDY

J. van den Kieboom et al.

Summary: In this study, the clinical outcomes of one-stage and two-stage exchange arthroplasty for chronic culture-negative PJI were compared. The results showed that there were no significant differences in reinfection, re-revision, readmission rates, mortality, and amputation between the two treatment strategies. This suggests that culture negativity may not be a contraindication to one-stage revision arthroplasty for chronic culture-negative PJI in selected patients.

Summary: The study indicates that early DAIR within 2 weeks of infection onset, using a standard surgical and postoperative protocol, can lead to favorable outcomes in the Indian population. Patient comorbidities, culture status, Gram staining characteristics, and identity of pathogenic bacteria did not influence the success of DAIR for acute PJI.

The change in plasma D-dimer does not help to guide the timing of reimplantation in two stage exchange for periprosthetic joint infection

Thomas Ackmann et al.

Summary: A study found that in a two-stage exchange for PJI, D-dimer levels did not change significantly, while CRP and IL-6 levels showed a decreasing trend. Some patients had an increase in D-dimer levels between stages, while most patients had decreasing levels of CRP and IL-6. Surgeons should exercise caution when using D-dimer to identify persisting infection during reimplantation.

Outcomes of single- and two-stage revision total knee arthroplasty for chronic periprosthetic joint infection LONG-TERM OUTCOMES OF CHANGING CLINICAL PRACTICE IN A SPECIALIST CENTRE

H. E. Matar et al.

Summary: Single-stage revision total knee arthroplasty (rTKA) is an effective strategy with a high success rate comparable to the two-stage approach in treating chronic per-prosthetic joint infections (PJIs). Factors such as the presence of a sinus and age over 80 years were identified as independent risk factors for failure. Patient survivorship at ten years was similar between the single-stage and two-stage groups.
