Controversy and Debate: Questionable utility of the relative risk in clinical research: Paper 1: A call for change to practice

Suhail A. Doi et al.

Summary: This article demonstrates the problem with using relative risk (RR) as a measure of effect magnitude in clinical trials. It suggests that odds ratio (OR) should be used instead, as it solely measures effect magnitude and is not influenced by the prevalence of an outcome. The author proposes to end the primary use of RR and replace it with OR for more meaningful interpretation of trial results.

Dose-effect meta-analysis for psychopharmacological interventions using randomised data

Tasnim Hamza et al.

Summary: This article describes the methods of conducting dose-effect meta-analysis using two-stage and one-stage models, and illustrates the methods on a dataset of selective serotonin reuptake inhibitor antidepressants. The results indicate that the probability of response increases with drug dose, but beyond a certain dose, there is no further increase in response. The one-stage model reduces uncertainty and should be used in practice.

Blood Pressure Effects of Sodium Reduction Dose-Response Meta-Analysis of Experimental Studies

Tommaso Filippini et al.

Summary: The study demonstrates an approximately linear relationship between dietary sodium intake and blood pressure, indicating that reducing sodium intake can lead to a decrease in blood pressure, with a more pronounced effect in individuals with hypertension.

Intention-to-treat analysis may be more conservative than per protocol analysis in antibiotic non-inferiority trials: a systematic review

Anthony D. Bai et al.

Summary: Contrary to common belief, ITT analysis was found to be more conservative than PP analysis in the majority of antibiotic non-inferiority trials, with a larger variance and wider confidence intervals. This finding suggests that ITT analysis should be considered as a mandatory analysis method in non-inferiority trials.

Empiric antibiotics for community-acquired pneumonia in adult patients: a systematic review and a network meta-analysis

Lara Montes-Andujar et al.

Summary: This network meta-analysis aimed to identify the empiric antibiotic with the highest probability of being the best in terms of cure rate and mortality rate in hospitalized patients with community-acquired pneumonia. Ceftaroline and piperacillin were found to be the best options for cure rate, while ceftriaxone plus levofloxacin, ertapenem, and amikacin plus clarithromycin were identified as the best options for mortality rate. Conducting randomized controlled trials comparing treatments with the highest probability of being the best for each event (cure or mortality) is deemed necessary.

Examination of Dosing of Antipsychotic Drugs for Relapse Prevention in Patients With Stable Schizophrenia A Meta-analysis

Stefan Leucht et al.

Summary: This meta-analysis examined dose-response findings in randomized clinical trials for antipsychotic drugs in preventing relapse in stable schizophrenia patients. The results indicated that doses higher than 5mg/d risperidone equivalent may not provide significant additional benefit for relapse prevention but may lead to more adverse events. On the other hand, patients in remission may require lower doses as low as 2.5mg/d risperidone equivalent. Individual factors such as metabolism, age, illness stage, and comorbidities may also influence the optimal dosage needed for each patient.
