Summary: The study mapped the landscape of clinical genomics in Australia holistically through the development of a rich picture, identifying key features, challenges, and leverage points of implementing clinical genomics using complex systems theory. The final graphic depicts 24 stakeholder groups relevant to the implementation of genomics in Australia, showing interplay between government, professional bodies, and patient advocacy groups, as well as barriers and uncertainties. Complexity theory analysis revealed far-reaching interdependencies around funding and unintended consequences, highlighting the need to understand key drivers and barriers to large-scale interventions.

It was found that patients' use of PtDA was influenced by competing medical conditions, life priorities, and socioeconomic context, while clinicians' use was impacted by pre-existing clinical tools, workflow, technical issues, clinic logistics, and time. A complexity lens was suggested to optimize the integration of PtDA into clinical care.
