Dolichoectasia of basilar artery presenting as non-communicating hydrocephalus-A rare presentation

Summary: A 67-year-old woman presented with symptoms of dizziness, difficulty walking, slowed gait, and disorientation for two days. Evaluation revealed hydrocephalus with cerebrospinal fluid leakage around the brain. Magnetic resonance angiography showed compression of the third ventricle by a dolichoectatic basilar artery, leading to reduced cerebrospinal fluid flow. Ventriculoperitoneal shunt insertion resulted in clinical improvement.

ETV is done using a zero-degree LOTTA endoscope, with the right Kocher's point chosen as the entry site into the ventricle, allowing communication between the third ventricle and various cisterns.
