Summary: Four randomized control trials were reviewed, involving 145 randomized participants who underwent either posterior-only or combined instrumentation for thoracolumbar burst fractures. There were no significant differences in outcomes such as postoperative kyphosis correction, pain scores, disability index, and postoperative complications between the two approaches. Posterior-only instrumentation was associated with lower blood loss, operative time, and length of stay, while combined instrumentation had a lower degree of kyphosis loss at final follow-up.
