Parathyroid 4D CT in Primary Hyperparathyroidism: Exploration of Size Measurements for Identifying Multigland Disease and Guiding Biochemically Successful Parathyroidectomy

Paul M. Bunch et al.

Summary: This study assesses the size of the second-largest lesion and individual gland size on parathyroid CT to differentiate single-gland disease from multigland disease. Optimal size thresholds are derived for successful parathyroidectomy.

4DCT Scanning Technique for Primary Hyperparathyroidism: A Scoping Review

Steven Raeymaeckers et al.

Summary: This study reviewed 51 articles and summarized 56 4DCT scanning protocols for the detection of parathyroid adenomas. Most protocols use three or four different phases, with three patterns of enhancement of parathyroid adenomas described. Contrast agent injection amount and timing showed some regularities in the protocols.

Diagnostic Value of Choline PET in the Preoperative Localization of Hyperfunctioning Parathyroid Gland(s): A Comprehensive Overview

Cristina Ferrari et al.

Summary: Hyperparathyroidism is a metabolic disorder characterized by excessive production of parathyroid hormone. Treatment often involves surgery, with advanced imaging techniques such as choline positron emission tomography showing promising results in accurately localizing hyperfunctioning parathyroid glands. Choline PET's higher sensitivity, better spatial resolution, and shorter examination times make it a potential first-line investigation for patients with inconclusive traditional imaging results.

Diagnostic Role of Four-Dimensional Computed Tomography for Preoperative Parathyroid Localization in Patients with Primary Hyperparathyroidism: A Systematic Review and Meta-Analysis

Lixin Sun et al.

Summary: Four-dimensional computed tomography (4D-CT) shows moderate sensitivity and specificity for preoperative localization of hyperfunctioning parathyroid glands (HPGs) in patients with primary hyperparathyroidism (pHPT). However, further research is needed to confirm and improve the diagnostic performance, especially when 4D-CT is considered for first-line use on a lesion-based level.

Summary: This study conducted a systematic review and meta-analysis, finding that the sensitivity of F-18-choline PET/CT and F-18-choline PET/4DCeCT is higher than that of 4DCeCT, while only a slight difference was observed between F-18-choline PET/CT and F-18-choline PET/4DCeCT.

Will18F-fluorocholine PET/CT replace other methods of preoperative parathyroid imaging?

Luca Giovanella et al.

Summary: Cervical ultrasound and parathyroid scintigraphy are commonly used in diagnosing PHPT, but four-dimensional CT and radiolabeled choline PET have emerged as useful alternatives when initial imaging methods are inconclusive. Radiolabeled choline PET/CT shows excellent diagnostic performance and low radiation exposure, making it an ideal candidate to replace current imaging procedures for preoperative localization of HP in patients with PHPT, including challenging cases of postoperative persistent/recurrent disease.
