Reduced HU in vertebral bodies and screw holding planes are independent risk factors for screw loosening. Cranial screws are at higher risk than caudal screws. Preoperative HU measurement and anti-osteoporosis therapy can effectively reduce the risk of screw loosening.

Fusion surgery can alter the biomechanics of adjacent discs, highlighting the importance of quantitative assessment of adjacent segment degeneration post-fusion.
