The ethical landscape(s) of non-invasive prenatal testing in England, France and Germany: findings from a comparative literature review

Adeline Perrot et al.

Summary: In this paper, the authors investigate how the introduction of NIPT into routine prenatal care has raised ethical concerns, and how these concerns are discussed differently across England, France, and Germany due to their unique socio-political particularities and value systems. The comparative review focuses on arguments and regulations surrounding prenatal testing and reproductive choices, shedding light on the varying definitions and implementations of reproductive autonomy in each country.

Clinical utility of expanded NIPT for chromosomal abnormalities and etiology analysis of cytogenetic discrepancies cases

Yue Hu et al.

Summary: Expanded NIPT shows promising performance in detecting chromosome abnormalities and CNVs, although the accuracy rate needs improvement. The occurrence of mitotic non-disjunction at different times in early embryonic development results in varying levels of chromosomal mosaicism in different placental and fetal tissues. The study underscores the significance of comprehensive cytogenetic validation of placental and fetal specimens with inconsistent NIPT results.

Initial Clinical Experience with NIPT for Rare Autosomal Aneuploidies and Large Copy Number Variations

Thomas Harasim et al.

Summary: This study evaluated the clinical outcome of non-invasive prenatal testing (NIPT) results positive for rare autosomal aneuploidies (RAA) and large copy number variations (CNVs). The study found that 50% of pregnancies with an NIPT-positive result for RAA showed signs of placental insufficiency or intra-uterine death, while the other 50% had a normal outcome at birth. Additionally, CNV testing by NIPT allowed for the detection of unbalanced translocations and relevant maternal health conditions.

Controversies in implementing non-invasive prenatal testing in a public antenatal care program COMMENT

Kjell Asmund Blix Salvesen et al.

Summary: Women's autonomy and inclusive society are highly valued in Norway. However, implementing non-invasive prenatal testing (NIPT) in public antenatal care program is challenging due to controversy over screening for trisomies and lower prevalence of fetal trisomies in younger women. Organizing the service between private institutions and public hospitals also raises debate and challenges.

Presence of cell-free fetal circRNA in maternal plasma

Yuxin Ran et al.

Summary: Efforts to find stable cell-free fetal RNA in maternal circulation for non-invasive prenatal testing (NIPT) have become urgent. This study discovered the presence of fetal circRNA in the peripheral blood of pregnant women, which persisted even after delivery for at least 24 hours. Furthermore, it was found that cff-circRNA might have specific expression patterns in gestational diseases. These findings may contribute to the development of NIPT.

Non-invasive PGT-A (niPGT-A) is a novel approach that analyzes genetic material in spent embryo culture media (SECM). However, niPGT-A has limitations including lower quantity and quality of cell-free genetic material and unknown origin.

Health economic evaluation of noninvasive prenatal testing and serum screening for down syndrome

Gefei Xiao et al.

Summary: This study analyzed the clinical data of DS screening and pregnancy outcomes in pregnant women in Zhuhai, China. The cost-effectiveness of serum screening (SS) and noninvasive prenatal testing (NIPT) for DS was evaluated. The results showed that NIPT, as a primary screening strategy, had the highest health economic value, reducing the frequency of invasive prenatal diagnosis (IPD) and associated miscarriages, saving medical resources, and significantly increasing the detection rate of DS.
