Molecular genetic evidence supporting diverse histogenic origins of germ cell tumors

Seung-Hyun Jung et al.

Summary: This study identified the histogenesis of GCTs through the analysis of single-nucleotide polymorphism array, revealing different genetic alterations between histologic areas within the same GCT. The findings suggest that GCTs may have various histogenesis and intratumoral genomic differences, which could be crucial for the identification of GCTs with different histologic areas.

Outcomes of intracranial germinoma-A retrospective multinational Asian study on effect of clinical presentation and differential treatment strategies

Kyung-Nam Koh et al.

Summary: This multinational study aimed to report clinical presentations and treatment strategies in patients with intracranial germinomas. The outcomes showed excellent survival rates, with chemotherapy having no impact on survival and radiotherapy field being associated with recurrence-free survival.

The Japan Society for Neuro-Oncology guideline on the diagnosis and treatment of central nervous system germ cell tumors

Hideo Nakamura et al.

Summary: Primary CNS germ cell tumors are rare neoplasms primarily seen in children and young adults. While treatment standardization remains challenging due to low case volume, advancements have been made in recent years through modified radiation and chemotherapy methods. In order to provide clinical management recommendations, the Japan Society for Neuro-Oncology established a multidisciplinary task force to create an evidence-based guideline for CNS GCTs.

Phase II trial of response-based radiation therapy for patients with localized germinoma: a Children's Oncology Group study

Ute Bartels et al.

Summary: This study evaluated the effectiveness of simplified chemotherapy followed by dose-reduced irradiation for the treatment of localized germinoma. The results showed high chemotherapy response rates, favorable survival rates, and potential for reducing long-term morbidities in patients with germinoma.

EANO, SNO and Euracan consensus review on the current management and future development of intracranial germ cell tumors in adolescents and young adults

Didier Frappaz et al.

Summary: The incidence of intracranial germ cell tumors is lower in European and North American populations compared to Asian populations. Treatment strategies focus on reducing neurological sequelae and are tailored based on markers and histological characteristics, with a combination of surgery, chemotherapy, and radiation therapy. Future strategies aim at decreasing long-term side effects while maintaining high cure rates, particularly for non-germinomatous germ cell tumors.

Roles of Tumor Markers in Central Nervous System Germ Cell Tumors Revisited with Histopathology-Proven Cases in a Large International Cohort

Hirokazu Takami et al.

Summary: This study investigates the optimal cut-offs for tumor markers in CNS germ cell tumors (GCTs) to specify non-germinomatous GCTs. It concludes that an integrated diagnosis based on tumor markers and histopathology is key to precise diagnosis and treatment strategies for this rare and heterogeneous tumor. The study also raises questions about the treatment approach for marker-positive germinomas or teratomas and optimal treatment strategy for marker-negative immature teratomas.

Endocrine manifestations of paediatric intracranial germ cell tumours: from diagnosis to long-term follow-up

Cristina Partenope et al.

Summary: This study examined endocrine manifestations in paediatric patients with IC-GCTs and found that diabetes insipidus, adrenal insufficiency, and hypothyroidism were common endocrine disorders. These disorders often occurred before tumour diagnosis and did not significantly affect survival rates. Lifelong surveillance is necessary to provide the best care for these patients.

Long-term survival in patients with primary intracranial germ cell tumors treated with surgery, platinum-based chemotherapy, and radiotherapy: a single-institution study

Hiroyuki Shimizu et al.

Summary: The study aimed to evaluate treatment outcomes and toxicities of patients with intracranial germ cell tumors (GCTs). Patients in the germinomatous group had significantly higher overall survival (OS) and progression-free survival (PFS) rates compared to those in the NGGCT group. Platinum-based chemotherapy and radiotherapy yielded favorable survival outcomes in germinomatous GCTs, while outcomes were unfavorable in the NGGCT intermediate prognosis and poor prognosis groups.

Long-Term Outcomes and Sequelae Analysis of Intracranial Germinoma: Need to Reduce the Extended-Field Radiotherapy Volume and Dose to Minimize Late Sequelae

Joo Ho Lee et al.

Summary: The study indicated that reducing the radiation dose in the extended field is crucial for decreasing late toxicities. For patients with complete response, upfront chemotherapy may help minimize radiation dose to 30 Gy. Prospective trials focusing on de-intensification of extended field radiation therapy are needed.

Precise detection of the germinomatous component of intracranial germ cell tumors of the basal ganglia and thalamus using placental alkaline phosphatase in cerebrospinal fluid

Kentaro Chiba et al.

Summary: The study showed that PLAP values can specifically identify the germinomatous component of IGCTs in the basal ganglia and thalamus with high sensitivity and specificity. PLAP is beneficial for the safe and timely detection of the germinomatous component, avoiding invasive surgical procedures and enabling prompt initiation of chemoradiation therapy.

Summary: The increased use of neuroimaging and the aging of society have changed the incidence and proportion of histological types of intracranial tumors in Japan. The study in Miyazaki Prefecture found that meningioma was the most common tumor, and the age-specific incidence rates of various tumors were similar between Miyazaki and Kumamoto.
