Impact of Three-Dimensional (3D) Visualization on Laparoscopic Hepatectomy for Hepatocellular Carcinoma

Summary: The impact of three-dimensional (3D) visualization on laparoscopic hepatectomy for hepatocellular carcinoma was investigated in this retrospective study. The results showed that the availability of 3D laparoscopy has led to a significant increase in the proportion of laparoscopic major hepatectomies and difficult resections. After propensity score matching, patients who underwent 3D laparoscopic hepatectomy had comparable operative time and blood loss with those who underwent 2D laparoscopic hepatectomy, but had lower overall complications. Furthermore, patients who underwent 3D laparoscopic major hepatectomy had a shorter hospital stay compared to those who underwent open surgery.

Preoperative 3D-virtual casts are helpful in predicting FLR, with a tendency to slightly underestimate the FLR, particularly in cases with more than 12 CLMs. The use of 3D-analysis for FLR estimation is feasible and leads to safer surgery with lower major complications in R1vasc-OSH.
