Procedure-Related Risk Factors for Surgical Site Infection in Dermatologic Surgery

Complications After Mohs Micrographic Surgery in Patients Aged 85 and Older

Summary: As the US population ages, safe surgical procedures are necessary for treatment of cutaneous neoplasms in very elderly patients. This study aimed to determine the incidence of complications associated with Mohs micrographic surgery (MMS) in patients aged 85 and older, and the risk factors that predispose to complications. Results showed that independent risk factors associated with a statistically higher incidence of complications were anticoagulant use, extremity location, greater than 2 MMS stages, and flap repair.

Characteristics of Lower Extremity Infection Rates Following Mohs Micrographic Surgery

Summary: The study evaluated lower extremity SSI rates post-MMS and found that wound closure with flaps/grafts significantly increased infection rates. More complicated repair techniques for larger wounds, such as flap/graft closure, contribute to increased SSI risk, suggesting providers guide antibiotic prophylaxis accordingly.

Invasive and in situ lesions of squamous cell carcinoma are independent factors for postoperative surgical-site infection after outpatient skin tumors surgery: A retrospective study of 512 patients

Summary: In surgical-site infections (SSI) after outpatient skin surgery for skin tumors, invasive squamous cell carcinoma, Bowen's disease, and actinic keratosis were identified as significant risk factors. Other factors associated with SSI included larger defect diameters, presence of ulcers, use of full-thickness skin graft and local skin flaps, medical history of diabetes mellitus, and use of immunosuppressive agents. However, in multivariate analysis, only invasive squamous cell carcinoma, Bowen's disease, and actinic keratosis retained significance.

Research suggests diabetes mellitus may be associated with increased infections, while immunosuppression is a mixed risk factor. Addition of antibiotics generally does not confer benefits, and Mohs micrographic surgery is proven safe both in office and inpatient settings.

Risk Factors for Infection After Minor Dermatologic Procedures: A Case-Control Study

A retrospective case-control study found that the infection rate after procedures using buffered lidocaine prepared in office was low. Procedures on the arm and leg showed a higher risk of infection compared to those on the head/neck.
