Effects of isolated maternal hypothyroxinemia on adverse pregnancy outcomes

Sima Nazarpour et al.

Summary: The study found that women with maternal hypothyroidism identified in early pregnancy are more likely to have adverse pregnancy outcomes, including preterm premature rupture of membranes (PPROM) and low birth weight (LBW), compared to women with euthyroid status. Additionally, neonatal head circumference and weight were lower in the hypothyroid group than in the euthyroid group.

Summary: This study aimed to evaluate the effects of L-T4 on maternal and perinatal outcomes in pregnant women with hypothyroxinemia. The results showed that L-T4 treatment significantly reduced the miscarriage rate and the proportion of newborns admitted to the NICU, but did not significantly reduce the incidence of hypertensive disorder of pregnancy.

Lower free thyroid hormone levels are associated with high blood glucose and insulin resistance and normalize with the metabolic improvement in type 2 diabetic mellitus with euthyroid

Liping Gu et al.

Summary: This study found that decreased levels of free thyroid hormones within the normal range were associated with high blood glucose and insulin resistance. Following MMC management, improvements in blood glucose and insulin resistance were observed alongside the restoration of low-normal thyroid function.

Summary: This study found that hypothyroxinemia during pregnancy was associated with an increased risk of preeclampsia-eclampsia, especially in women with persistent hypothyroxinemia in early pregnancy. However, there was no significant association between hypothyroxinemia and gestational hypertension.

Early Levothyroxine Treatment for Subclinical Hypothyroidism or Hypothyroxinemia in Pregnancy: The St Carlos Gestational and Thyroid Protocol

Summary: The optimal maternal levels of thyroid hormones during the first trimester have not been established, but studies suggest that at gestational week 8, TSH <2.5 mu IU/mL and FT4 >7.5 pg/mL correspond to the 75.8% and 17.9% percentiles, respectively. Early initiation of levothyroxine treatment is beneficial for improving gestation progression in pregnant women with suboptimal TSH/FT4 levels.

Summary: This study found that isolated maternal hypothyroxinemia (IMH) was associated with adverse pregnancy outcomes, including lower birth weight, intrauterine growth restriction, lower Apgar scores, and increased likelihood of cesarean section. Additionally, TSH, TT4, and BMI were identified as strong predictors of perinatal outcomes.
