The contemporary pulmonary artery catheter. Part 2: measurements, limitations, and clinical applications

I. T. Bootsma et al.

Summary: The classical PAC has a long history of clinical use, evolving into a monitoring tool that provides continuous data on cardiac output, oxygen supply-demand balance, and right ventricular performance. This review discusses the difference between intermittent thermodilution and contemporary PAC measurements, highlighting the importance of understanding measurements for successful application in clinical practice.

Modified cardiovascular SOFA score in sepsis: development and internal and external validation

Hui Jai Lee et al.

Summary: The Sepsis-3 criteria introduced the use of the Sequential Organ-Failure Assessment (SOFA) score to define sepsis. This study aimed to develop and validate a modified Cardiovascular SOFA (CV SOFA) score, considering changes in recommended vasopressors. The modified CV SOFA score showed higher predictive validity and identified more patients at risk of septic mortality among patients with suspected infection, sepsis, and septic shock.

Right ventricular outflow tract Doppler flow analysis and pulmonary arterial coupling by transthoracic echocardiography in sepsis: a retrospective exploratory study

Emma Maria Bowcock et al.

Summary: This study investigated the feasibility and utility of combined analysis with RVOT Doppler and RV-PA coupling in assessing RV function in sepsis. The results showed that raised pulmonary vascular resistance and RV-PA uncoupling are common in sepsis patients. These parameters have potential clinical value in improving the definition, treatment, and prognosis of RV dysfunction.

Right Ventricular Dysfunction in Early Sepsis and Septic Shock

Summary: This study found that right ventricular dysfunction is common in septic patients and is associated with over threefold higher 28-day mortality. LV systolic dysfunction, on the other hand, was not found to be significantly associated with increased mortality.

New-Onset Atrial Fibrillation in Sepsis: A Narrative Review

Jesus Aibar et al.

Summary: Atrial fibrillation is a common arrhythmia in sepsis patients, with a mean incidence of 20.6%. Risk factors for new-onset AF in sepsis include advanced age, white race, male sex, obesity, and history of cardiopulmonary disease. In-hospital mortality is higher in patients with new-onset AF, and there is an increased risk of stroke associated with this condition. More large prospective studies are needed to determine the necessity of anticoagulation in sepsis patients with new-onset AF.

Predicting mortality with cardiac troponins: recent insights from meta-analyses

Giuseppe Lippi et al.

Summary: The introduction of cardiac troponin testing has been a crucial advancement in clinical practice, essential for diagnosing acute coronary syndrome. Studies have shown that cTn values can predict mortality risk in both healthy individuals and patients with a variety of cardiac and non-cardiac diseases.
