Federated Learning for Privacy Preservation in Smart Healthcare Systems: A Comprehensive Survey

Summary: Recent advances in electronic devices and communication infrastructure have transformed the traditional healthcare system into a smart healthcare system through the use of internet of medical things (IoMT) devices. However, privacy issues arise with mobile and wearable IoMT devices due to the centralized training approach of artificial intelligence (AI) and the potential exposure of highly confidential information communicated between hospitals and end-users. To address this, federated learning (FL) has emerged as a distributed AI paradigm that allows privacy preservation in IoMT without accessing participants' confidential data. FL also provides privacy to end-users by sharing only gradients during training. This paper explores the privacy-related issues in IoMT, discusses the role of FL in preserving privacy in IoMT networks, introduces advanced FL architectures incorporating deep reinforcement learning (DRL), digital twin, and generative adversarial networks (GANs) for privacy threat detection, and presents practical opportunities for FL in IoMT. The survey concludes by addressing open research issues and challenges in using FL in future smart healthcare systems.
