Visually sensitive seizures: An updated review by the Epilepsy Foundation

Robert S. Fisher et al.

Summary: Light stimulation can trigger seizures in certain individuals, especially in young people and those with specific forms of epilepsy. Photosensitive seizures can coexist with spontaneous seizures and have a genetic component. The exact mechanisms of photosensitive seizures are still unclear, but specific triggers such as flashes or color changes can provoke seizures. Prevention strategies include avoiding triggers, covering one eye, keeping a safe distance from screens, reducing contrast, and taking certain antiseizure medications. Visually-induced seizures pose significant public health risks, requiring ongoing scientific research, regulation, and public education.

Efficacy of levetiracetam in STXBP1 encephalopathy with different phenotypic and genetic spectra

Qiu-Hong Wang et al.

Summary: This study analyzed the prognosis of Levetiracetam (LEV) treatment for STXBP1 encephalopathy and explored the correlation among genotype, phenotype, and LEV efficacy. The results showed that LEV could improve the partial remission rate in patients with STXBP1 encephalopathy, but did not achieve seizure freedom. LEV combined with other anti-seizure medications could increase the seizure remission rate, especially for refractory types of epilepsy.

Epilepsy Genetics and Precision Medicine in Adults: A New Landscape for Developmental and Epileptic Encephalopathies

Alvaro Beltran-Corbellini et al.

Summary: This review focuses on epilepsy genetics and precision medicine in adult patients, particularly developmental and epileptic encephalopathies (DEEs). It discusses issues such as candidate selection for genetic testing, genetic test options, and interpretation. DEEs are clinically heterogeneous syndromes with challenges in diagnosis and treatment. Recent studies suggest that individuals with epilepsy and various risk factors are suitable candidates for genetic testing. An accurate diagnosis is crucial for precision medicine.

Summary: This study compared the efficacy and effects of oxcarbazepine and levetiracetam as monotherapies for patients with newly diagnosed focal epilepsy in China. The results showed that oxcarbazepine monotherapy may be more effective than levetiracetam monotherapy, and both drugs improved the quality of life and anxiety state of the patients.

Nicola Specchio et al.

Summary: The 2017 International League Against Epilepsy classification defines a three-tier system for epilepsy syndromes, with childhood onset epilepsy syndromes falling into three categories: self-limited focal epilepsies, generalized epilepsies, and developmental and/or epileptic encephalopathies. Identification of specific syndromes helps guide management and prognosis. This paper describes these childhood onset epilepsy syndromes, highlighting their seizure types, EEG features, phenotypic variations, and key investigation findings.

A pharmacogenomic assessment of psychiatric adverse drug reactions to levetiracetam

Ciaran Campbell et al.

Summary: This study aimed to determine the contribution of genetic variation to psychiatric and behavioral adverse drug reactions associated with levetiracetam. The results showed that a polygenic burden for schizophrenia is a risk factor for levetiracetam-associated psychotic reaction, but no evidence of an increased burden of rare genetic variants was found.

Trends in antiseizure medication prescription patterns among all adults, women, and older adults with epilepsy: A German longitudinal analysis from 2008 to 2020

Maja Hochbaum et al.

Summary: This study analyzed the patterns of antiseizure medication prescriptions among adult epilepsy patients and found an increasing trend in the prescription of third-generation drugs. The use of first- and second-generation drugs decreased, and enzyme-inducing drugs were used less frequently. Among women of childbearing potential, prescriptions were adjusted to minimize the risk of birth defects. Older adults showed a decrease in the use of carbamazepine to avoid hyponatremia risks and drug interactions. Levetiracetam was commonly used as a monotherapy for patients with genetic generalized epilepsy.

Levetiracetam for Seizure Prophylaxis in Neurocritical Care: A Systematic Review and Meta-analysis

Summary: Based on current data with moderate to high risk of bias, the use of levetiracetam prophylaxis for seizure prevention in TBI, SAH, and ICH did not show significant reductions in seizure incidence. However, it may be beneficial for patients after supratentorial neurosurgery. More high-quality randomized trials are needed to further evaluate the prophylactic use of levetiracetam.

Concentrations of antiseizure medications in breast milk of lactating women with epilepsy: A systematic review with qualitative synthesis

Ramzi Shawahna et al.

Summary: This systematic review evaluated the concentrations of antiseizure medications (ASMs) in the breastmilk of women with epilepsy (WWE) and assessed the potential risks to infants. The results revealed that some ASMs had high relative infant doses, which could have adverse effects on infants. Therefore, close monitoring of infants' conditions and consideration of reducing or discontinuing breastfeeding should be applied during lactation.

Synaptopathies in Developmental and Epileptic Encephalopathies: A Focus on Pre-synaptic Dysfunction

Giulia Spoto et al.

Summary: The proper connection between pre- and post-synaptic nervous cells relies on various components including membranes, the synaptic cleft, glial cells, and extracellular matrix. Synaptopathy refers to the alteration of mechanisms regulating these synaptic components and is associated with neuropsychiatric disorders. Developmental and Epileptic Encephalopathies (DEEs) are a group of epilepsy syndromes associated with cognitive disturbances, intellectual disability, autistic features, and movement disorders. This review focuses on the pathogenesis of DEEs, particularly on the role of pre-synaptic terminals and potential therapeutic approaches.

Alternatives to valproate in girls and women of childbearing potential with Idiopathic Generalized Epilepsies: state of the art and guidance for the clinician proposed by the Epilepsy and Gender Commission of the Italian League Against Epilepsy (LICE)

Barbara Mostacci et al.

Summary: In response to recent restrictions on valproate use in females of childbearing potential, the Italian League against Epilepsy's Commission on Epilepsy and Gender synthesized current literature and legislative data to guide clinicians on antiseizure medication prescription for Idiopathic Generalized Epilepsies, recommending alternatives to valproate. The review highlighted first-choice drugs such as levetiracetam and lamotrigine in specific epilepsy syndromes, while also addressing off-label use, contraindications, and discrepancies in warnings among different brands of the same active ingredient. Recommendations include the need for revised prescription rules consistent with latest literature data and uniformity in warnings for the same active ingredients.
