Identification of critical control points for antibiotic resistance discharge in sewers

Summary: Disrupting the spread of clinically relevant antibiotic resistance genes (ARGs) is crucial for the success of the One Health strategy. This study focuses on monitoring wastewater feeds of selected urban drainage areas, and identifies hospitals and retirement homes as major sources of high levels of ARGs in wastewater. The monitoring approach includes qPCR-based quantifications, flow volume-based analyses, and antibiotic resistance analyses. The study suggests that local actions at critical control points can prevent contamination of larger volumes of wastewaters in a cost-effective manner, compared to central actions at wastewater treatment plants (WWTPs). The implementation of a polluter-pays principle is recommended for this monitoring strategy.

IncFIB plasmids carrying the resistance gene blaCTX-M-15 in ESBL-producing Escherichia coli clones from pediatric patients

CTX-M-15-producing Klebsiella pneumoniae ST273 associated with nasal infection in a domestic cat

Detection of NDM-1/5 and OXA-48 co-producing extensively drug-resistant hypervirulent Klebsiella pneumoniae in Northern Italy

The results revealed the presence of new high-risk K. pneumoniae clones that co-produced OXA-48, NDM-1, and NDM-5, posing a significant challenge to public health.

Distribution and characteristics of carbapenem-resistant and extended- spectrum β-lactamase (ESBL) producing Escherichia coli in hospital effluents, sewage treatment plants, and river water in an urban area of Japan

Summary: The occurrence of carbapenem-resistant Escherichia coli (CRE-E) and extended-spectrum beta-lactamase (ESBL) producing Escherichia coli (ESBL-E) in an urban river in Japan was investigated. The study found that the wastewater discharges from a sewage treatment plant had a significant contribution to the pollution load in the river water, and the presence of antimicrobial-resistant bacteria (AMRB) in the river was linked to the prevalence of AMRB in clinical settings. These findings highlight the importance of environmental risk management of AMRB and antimicrobial-resistance genes (AMRGs) in river environments.

Prevalence of enteric opportunistic pathogens and extended-spectrum cephalosporin- and carbapenem-resistant coliforms and genes in wastewater from municipal wastewater treatment plants in Croatia

Summary: Extended-spectrum beta-lactamase (ESBL) and carbapenemase-producing Enterobacterales are still present in the environment even after conventional treatment in wastewater treatment plants. Enteric opportunistic pathogens such as Klebsiella pneumoniae and Acinetobacter baumannii are efficiently removed, but Escherichia coli and Enterococcus spp. still remain in relatively high concentrations in the effluent. Improvement in wastewater treatment technologies is necessary to minimize the risk of environmental contamination and public health issues related to ESBL and carbapenemase-producing Enterobacterales.
