Ketamine for Refractory Chronic Migraine: An Observational Pilot Study and Metabolite Analysis

Summary: Patients with refractory chronic migraine who have failed multiple medications may benefit from aggressive intravenous therapy using lidocaine and (R,S)-ketamine infusions, as both have shown short-term analgesic effects. Analysis of ketamine metabolites revealed that (2R,6R)-hydroxynorketamine is the primary metabolite during infusion, consistent with previous studies. While ketamine showed a slightly greater decrease in pain compared to lidocaine, the clinical significance of this difference is minimal.
