Summary: Performing VAS surgery with a Somatex shunt before 17 + 0 weeks gestational age significantly improves perinatal survival and may positively impact renal function, likely due to lower dislocation rates. A normal amount of amniotic fluid in the third trimester is the best predictor of normal renal function in early childhood.

Improving the accuracy of prenatal diagnosis and identifying potential risk factors for fetal and postnatal renal failure are essential. When selecting the timing for intervention, various factors need to be considered, especially if the goal is renal preservation.
