Role of ephaptic coupling in discordant alternans domain sizes and action potential propagation in the heart

Niels F. Otani et al.

Summary: Discordant alternans, the spatially out-of-phase alternation of action potentials durations in the heart, is linked to the onset of fibrillation. Computer models have struggled to reproduce the small domain sizes and rapid action potential propagation speeds seen in experiments. This study shows that a more detailed model of intercellular coupling that accounts for ephaptic effects can achieve rapid wave speeds and small domain sizes.

Simulation of atrial fibrillation in a non-ohmic propagation model with dynamic gap junctions

Ariane Saliani et al.

Summary: This study used a left atrial model to simulate episodes of arrhythmias and quantified the impact of nonlinear diffusion on propagation patterns. The results show that the linear approximation is valid for faster conduction velocities, but nonlinear diffusion leads to qualitative differences in propagation patterns in slower conduction substrates.

Computational modeling of aberrant electrical activity following remuscularization with intramyocardially injected pluripotent stem cell-derived cardiomyocytes

Joseph K. Yu et al.

Summary: EAs may arise from the automaticity of immature pluripotent stem cell-derived cardiomyocytes in nascent myocardial grafts. During early engraftment, alterations in impulse formation and propagation may occur. Therefore, in addition to focal mechanisms, other factors may contribute to EAs.

Mechanisms underlying age-associated manifestation of cardiac sodium channel gain-of-function

Madison B. Nowak et al.

Summary: Cardiac action potentials are initiated by Na+ influx through Na+ channels, and GOF in these channels can lead to arrhythmias. Localization of cardiac Na+ channels at the intercalated disc facilitates EAD formation. Variability in cellular properties at different ages may lead to EADs presenting at early developmental stages.

Intercalated disk nanoscale structure regulates cardiac conduction

Nicolae Moise et al.

Summary: The intercalated disk in the heart plays an important role in regulating cardiac conduction by affecting the distribution of Na' channels and electrical currents. Studies show that disruptions in intercalated disk structure can alter cardiac conduction. Modeling based on finite element analysis suggests that heterogeneities in intercalated disk structure influence electrical polarization and conduction within the intercellular cleft, impacting the activation of Na' currents and conduction velocity.

Localization of Na+ channel clusters in narrowed perinexi of gap junctions enhances cardiac impulse transmission via ephaptic coupling: a model study

Summary: It has been suggested that the localization of Na+ channel clusters in the perinexi of gap junction plaques is crucial for ephaptic coupling in cardiac tissue, which is greatly modulated by perinexal width.

Quantifying arrhythmic long QT effects of hydroxychloroquine and azithromycin with whole-heart optical mapping and simulations

Ilija Uzelac et al.

Summary: The study found that HCQ alone or in combination with AZM can lead to long QT intervals, prolong the duration of action potential in the heart, and increase spatial dispersion of action potential repolarization, resulting in proarrhythmic discordant alternans. AZM alone has a lesser arrhythmic effect with less triangulation of the action potential shape. Mathematical cardiac models failed to reproduce most of the arrhythmic effects observed in experiments.
