Summary: High intensity focused ultrasound (HIFU) can rapidly and non-invasively destroy tumor tissue. A combination of HIFU with innate immune agonist CpG and checkpoint inhibitor anti-PD-1 can modulate the immune response, leading to enhanced anti-tumor effects. Both mechanical disruption HIFU and thermal ablation induce potent inflammatory responses, while priming with CpG can further enhance immune response, alter macrophage polarization, and increase T-cell activation.

Summary: The study investigated the efficacy of focused ultrasound based boiling histotripsy (HT) and in-situ anti-CD40 agonist antibody (alpha CD40) combinatorial therapy in enhancing therapeutic efficacy against immune checkpoint inhibitor (ICI) refractory murine melanoma. Results showed that HT40 treatment upregulated inflammatory markers in tumors, increased population of functional CD8+ T cells, M1 to M2 macrophage ratio, and CD8+ T: regulatory T cell ratio. Additionally, the combination of HT40 and ICI therapy showed superior inhibition of distant untreated tumors and prolonged survival rates.
