Persistence Pays Off: Milvexian Emerges from the Industry's Longstanding Search for Orally Bioavailable Factor XIa Inhibitors

Nicole C. Goodwin

Summary: Factor XIa inhibitors have emerged as a new class of antithrombic agents in the past two decades, with the identification of orally bioavailable small molecule inhibitors posing a significant challenge for medicinal chemists. The discovery of milvexian resulted from problem-solving efforts to overcome these challenges.

Discovery of Milvexian, a High-Affinity, Orally Bioavailable Inhibitor of Factor XIa in Clinical Studies for Antithrombotic Therapy

Andrew K. Dilger et al.

Summary: FXIa, an enzyme in the coagulation cascade, has limited role in hemostasis but is thought to amplify thrombin generation. Inhibitors of FXIa have potential as antithrombotic agents with superior efficacy and safety. The discovery of a novel series of macrocyclic FXIa inhibitors containing a pyrazole P2' moiety has led to the identification of milvexian as a clinical candidate for the prevention and treatment of thromboembolic disorders, suitable for oral administration.

Scale-Up of Diazonium Salts and Azides in a Three-Step Continuous Flow Sequence

Hansjoerg Lehmann et al.

Summary: The rapid synthesis and scale-up of active molecules is crucial for the development of new drug candidates in the pharmaceutical industry. This study describes a scalable continuous flow procedure for synthesizing 2H-indazoles, which are highly potent TLR7 and TLR8 antagonists. By transforming hazardous reactions from batch mode to continuous flow mode, the risks associated with handling large amounts of hazardous reagents and intermediates were mitigated and limited.
