Relevance of Excessive Air Swallowing in GERD Patients With Concomitant Functional Dyspepsia and Poor Response to PPI Therapy

Mentore Ribolsi et al.

Summary: This study aimed to evaluate the role of air swallowing in the association between GERD symptoms and concomitant FD, as well as their role in GERD symptom persistence despite PPI therapy. It was found that individuals with excessive air swallowing experience FD, and GERD patients not responding to PPI therapy swallow more air during mealtime and also have more mixed refluxes.

The role of mood state and emotion regulation in the discrepancy between gastrointestinal symptom burden recorded prospectively and via recall questionnaire

Summary: Reports of gastrointestinal symptoms obtained via recall are likely to be exaggerated relative to the actual patient experience, particularly among healthcare seekers. The magnitude of the discrepancy is associated with patients reporting a diagnosis of a gastrointestinal condition and correlates with psychological traits.

Chronic stress increases DNA methylation of the GR promoter in the central nucleus of the amygdala of female rats

Tijs Louwies et al.

Summary: Irritable bowel syndrome (IBS) is a gastrointestinal disorder characterized by abdominal pain and abnormal bowel habits, but the underlying pathophysiological mechanisms are poorly understood. This study found that chronic stress can lead to increased activity in the amygdala, a stress-responsive brain region, in IBS patients. In addition, the experiment found that stress can alter DNA methylation levels in the central nucleus of the amygdala in rats, and the administration of HDAC inhibitors can prevent these changes.

Advancements in the use of 24-hour impedance-pH monitoring for GERD diagnosis

Elisa Marabotto et al.

Summary: Gastro-esophageal reflux disease (GERD) affects approximately 25% of the general population. Due to the complexity of the disease, there is no singular diagnostic test available. Most GERD patients do not have visible lesions, known as non-erosive reflux disease (NERD). Currently, 24-hour impedance-pH monitoring is the best diagnostic tool to detect abnormal reflux and correlate symptoms. Novel impedance metrics have shown potential to improve the accuracy of diagnosis.

Novel impedance-pH parameters are associated with proton pump inhibitor response in patients with inconclusive diagnosis of gastro-oesophageal reflux disease according to Lyon Consensus

Mentore Ribolsi et al.

Summary: The study examined the diagnostic yield of MNBI and PSPW index in patients with inconclusive GERD diagnosis and their relationship with PPI response. Abnormal MNBI and PSPW index were significantly more frequent in patients with inconclusive GERD, with those displaying pathological values more likely to respond positively to PPI treatment. This highlights the value of MNBI and PSPW index as adjunctive metrics in characterising patients with inconclusive evidence of GERD and identifying those responsive to PPI treatment.

Oesophageal hypervigilance and visceral anxiety relate to reflux symptom severity and psychological distress but not to acid reflux parameters

Ming-Wun Wong et al.

Summary: This study aimed to investigate the interrelationship between the oesophageal hypervigilance and anxiety scale (EHAS) and various factors in patients with gastro-oesophageal reflux disease (GERD). Results showed that EHAS was significantly correlated with GERDQ, questionnaires of depression, and anxiety. However, there were no significant correlations between GERDQ and questionnaires of depression or anxiety, indicating the potential of EHAS assessment in evaluating subjective patient outcomes and treatment satisfaction.
