36-month clinical outcomes of patients with venous thromboembolism: GARFIELD-VTE

Alexander G. G. Turpie et al.

Summary: GARFIELD-VTE is an observational study that aims to collect clinical data of patients with deep vein thrombosis and pulmonary embolism worldwide. The study found that the highest accumulation of events occurred within the first 12 months after diagnosis, which may help identify treatment gaps and improve patient outcomes.

Acute outcomes for the full US cohort of the FLASH mechanical thrombectomy registry in pulmonary embolism

Catalin Toma et al.

Summary: The acute safety and effectiveness of mechanical thrombectomy for intermediate-and high-risk PE were evaluated in a large real-world population using the FlowTriever System. The results demonstrated a favorable safety profile, improvements in hemodynamics and functional outcomes, and low 30-day mortality for intermediate-and high-risk PE.

Reduced-Dose Intravenous Thrombolysis for Acute Intermediate-High-risk Pulmonary Embolism: Rationale and Design of the Pulmonary Embolism International THrOmbolysis (PEITHO)-3 trial

Olivier Sanchez et al.

Summary: The PEITHO-3 study aims to compare the efficacy and safety of reduced-dose thrombolysis with standard anticoagulation in patients with intermediate-high-risk PE. The primary outcome is a composite of all-cause death, hemodynamic decompensation, or PE recurrence within 30 days, with key secondary outcomes including bleeding events. The study plans to enroll 650 patients and may impact future guideline recommendations for acute PE treatment.

Summary: This study aimed to investigate the real-world use of the Penumbra Indigo aspiration system. A total of 2118 reports were collected, and 67 reports related to device failure were analyzed. Lightning unit malfunction was the most common failure mode. Three deaths were reported, with two cases being attributed to severe complications.

Chronic thromboembolic pulmonary hypertension and impairment after pulmonary embolism: the FOCUS study

Summary: This study systematically assessed the late outcomes of acute pulmonary embolism (PE) and investigated the clinical implications of post-PE impairment (PPEI). The results showed that the cumulative incidence of chronic thromboembolic pulmonary hypertension (CTEPH) was 2.3%, while the incidence of PPEI diagnosed by standardized criteria was 16.0%. The presence of PPEI increased the risk of re-hospitalization and death, as well as worsened quality of life.

Pulmonary Embolism

Susan R. Kahn et al.

Summary: A 41-year-old male patient presents with a 3-week history of breathlessness. Physical examination and chest X-ray are unremarkable. The physician suspects a likelihood of pulmonary embolism greater than 15%. The patient's Wells score is 0 and D-dimer level is elevated. Evaluation and management of the patient's risk for pulmonary embolism are now required.

Riyaz Bashir et al.

Summary: This study evaluated the efficacy and safety of a pharmacomechanical catheter-directed thrombolysis (CDT) device called the Bashir endovascular catheter in patients with intermediate-risk acute pulmonary embolism (PE). The results showed a significant reduction in the RV/LV diameter ratio, PA obstruction, and low rates of adverse events or major bleeding.

The results showed that cCDT had some improvements in efficacy outcomes and an acceptable rate of major bleeding.

Outcome of patients with different clinical presentations of high-risk pulmonary embolism

Summary: The revised ESC 2019 guidelines definition of high-risk PE stratifies subgroups at different risk of in-hospital adverse outcomes and all-cause mortality. Risk prediction can be improved by using an optimised venous lactate cut-off value to diagnose obstructive shock, which might help to better assess the risk-to-benefit ratio of systemic thrombolysis in different subgroups of high-risk patients.
