Summary: This study evaluates the early effects of the acuity circle (AC)-based model on liver allocation and waitlist outcomes. The results show an increase in transplantation and waitlist mortality risks in the post-AC era, especially in high-MELD centers. Low-MELD centers had a higher proportion of donations after circulatory death (DCDs). Overall, the AC-based model may improve waitlist outcomes, but there is regional variation in the drivers of these outcomes between centers.
