Functional alignment achieves a more balanced total knee arthroplasty than either mechanical alignment or kinematic alignment prior to soft tissue releases

Gavin Clark et al.

Summary: This study assessed the balance achievable with mechanical alignment and kinematic alignment plans, and found that functional alignment obtained significantly better balance. Overall balance was observed in 97% of functional alignment knees, 73% of kinematic alignment knees, and in 55% of mechanical alignment knees. Utilizing an individualized kinematic alignment plan allows functional alignment to best achieve the goals of maintaining joint line plane and obliquity.

Osteoarthritic and non-osteoarthritic patients show comparable coronal knee joint line orientations in a cross-sectional study based on 3D reconstructed CT images

Silvan Hess et al.

Summary: The study aimed to assess the alignment of a large series of osteoarthritic knees and compare it to non-OA knees, finding small differences in the coronal femoral and tibial joint line orientation between the two groups. The results suggest that femoral and tibial joint line orientation of osteoarthritic patients can be used to estimate their native coronal alignment for individualized knee alignment planning.

Robot-assisted knee arthroplasty improves component positioning and alignment, but results are inconclusive on whether it improves clinical scores or reduces complications and revisions: a systematic overview of meta-analyses

Nanne Kort et al.

Summary: Robot-assisted knee arthroplasty allows for more accurate component positioning but significantly prolongs operation time. While robotic assistance improves component alignment, its benefits in terms of clinical scores, patient satisfaction, and implant survivorship need further validation.

Association of a Wider Medial Gap (Medial Laxity) in Flexion with Self-Reported Knee Instability After Medial-Pivot Total Knee Arthroplasty

Hideki Ueyama et al.

Summary: This study investigated the association between joint gap imbalance in flexion and subjective knee instability, as well as the influence of subjective knee instability on outcomes following medial-pivot total knee arthroplasty (TKA). The results showed that postoperative flexion joint-gap laxity on the medial side affected patient-reported subjective knee instability scores. Furthermore, patients with subjective knee instability had poorer subjective scores following medial-pivot TKA.

Wide variation in tibial slopes and trochlear angles in the arthritic knee: a CT evaluation of 4116 pre-operative knees

Kaushik Hazratwala et al.

Summary: This study evaluates the variability in knee anatomy and alignment beyond the coronal plane, highlighting the overemphasis in the literature on coronal alignment. The findings suggest that existing arthroplasty techniques may not adequately address the considerable variability in tibial and patellofemoral morphology, leading to potential dissatisfaction among patients.

Functional alignment with robotic-arm assisted total knee arthroplasty demonstrated better patient-reported outcomes than mechanical alignment with manual total knee arthroplasty

Byung Sun Choi et al.

Summary: This study compared the accuracy and short-term clinical outcomes of robot-assisted functionally aligned total knee arthroplasty (RFA-TKA) with mechanically aligned total knee arthroplasty (MA-TKA). The results showed that RFA-TKA had greater accuracy in implant positioning and better patient-reported outcomes compared to MA-TKA.

Arithmetic hip-knee-ankle angle and stressed hip-knee-ankle angle: equivalent methods for estimating constitutional lower limb alignment in kinematically aligned total knee arthroplasty

Payam Tarassoli et al.

Summary: This study compared the arithmetic HKA and stressed HKA measurements using radiographs and computer-assisted navigation, and found no significant difference between the two methods. These findings further support the use of the preoperative arithmetic method and the intraoperative stressed HKA technique in restoring constitutional lower limb alignment in KA TKA.

Functional alignment achieves soft-tissue balance in total knee arthroplasty as measured with quantitative sensor-guided technology

J. S. Chang et al.

Summary: This study assessed the effect of TKA with functional alignment on mediolateral soft-tissue balance using intraoperative sensor-guided technology. The results showed that TKA with functional alignment achieved balanced mediolateral soft-tissue tension through the arc of knee flexion. Further clinical trials are needed to assess whether this approach leads to improved patient satisfaction and outcomes compared to traditional alignment techniques.

Coronal Plane Alignment of the Knee (CPAK) classification A NEW SYSTEM FOR DESCRIBING KNEE PHENOTYPES

S. J. MacDessi et al.

Summary: The study introduces the Coronal Plane Alignment of the Knee (CPAK) classification, which includes nine phenotypes based on the arithmetic HKA that estimates constitutional limb alignment and joint line obliquity (JLO). In a cohort of 138 computer-assisted TKAs, a greater proportion of kinematic alignment (KA) TKAs achieved optimal balance compared to mechanical alignment (MA), with statistically significant differences seen in CPAK Types I, II, and IV.
