The pathogenesis of ischemic stroke in COVID-19 patients is still uncertain, but evidence suggests it may be related to cytokine storm-induced coagulopathy and endotheliopathy. Specific management challenges include difficulty in identifying stroke in critically ill patients and the need for a protected pathway for brain imaging and thrombectomy.

Stroke Mechanism in COVID-19 Infection: A Prospective Case- Control Study

However, no significant differences were found in terms of antiphospholipid antibodies and hereditary thrombophilia between COVID-19 positive and negative patients.
