Pitfalls and Common Findings in 68Ga-FAPI PET: A Pictorial Analysis

Lukas Kessler et al.

Summary: This study summarizes common findings and potential pitfalls in Ga-68-FAPI PET/CT. Common findings include uptake in degenerative lesions and head-and-neck. Pitfalls include non-tumor-specific uptake in degenerative lesions, muscle, scarring, mammary glands, or uterus.

Initial Clinical Experience with 90Y-FAPI-46 Radioligand Therapy for Advanced-Stage Solid Tumors: A Case Series of 9 Patients

Justin Ferdinandus et al.

Summary: This study investigates the feasibility and safety of Y-90-FAPI-46 radioligand therapy in patients with solid tumors. The results demonstrate good tolerability and a low rate of adverse events. Furthermore, the low radiation doses to at-risk organs suggest the possibility of repeat cycles of Y-90-FAPI-46.

Safety and Efficacy of 90Y-FAPI-46 Radioligand Therapy in Patients with Advanced Sarcoma and Other Cancer Entities

Wolfgang P. Fendler et al.

Summary: This study reports the efficacy and safety of 90Y-labeled FAPI-46 in patients with advanced sarcoma, pancreatic cancer, and other cancers. The results show disease control in approximately 38% of patients, with a prolonged overall survival associated with disease control. However, there were adverse events observed, with thrombocytopenia and anemia being the most common.

Non-malignant findings of focal 68Ga-FAPI-04 uptake in pancreas

Xiao Zhang et al.

Summary: Although Ga-68-FAPI-04 shows great potential for oncological diagnosis, focal uptake of Ga-68-FAPI-04 may occur in benign pancreatic lesions and should be carefully evaluated. Fortunately, Ga-68-FAPI-04 PET/MR, when combined with MR imaging, may potentially help avoid misdiagnosis of certain pancreatic lesions.

Equivalent tumor detection for early and late FAPI-46 PET acquisition

J. Ferdinandus et al.

Summary: The study compared early (approximately 10 min p.i.) and late (approximately 60 min p.i.) FAPI-46 imaging in patients with various types of cancer. There was equivalent lesion uptake and tumor detection between the two time points. Early FAPI-46 PET imaging was recommended for improved feasibility and scan volume in future clinical and research protocols.

False-Positive 68Ga-Fibroblast Activation Protein-Specific Inhibitor Uptake of Benign Lymphoid Tissue in a Patient With Breast Cancer

Cihan Gundogan et al.

Summary: In a patient with left lower outer quadrant breast cancer, Ga-68-FAPI-04 PET/CT showed higher accumulation in the primary tumor and axillary lymph nodes than F-18-FDG PET/CT. Despite focal increased FAPI uptake in another nodular lesion being considered malignant, the Tru-Cut biopsy revealed it as benign lymphoid tissue. This case highlights the importance of histopathological confirmation for interpreting FAPI accumulation in breast tissue.

Increased 68Ga-FAPI-04 Uptake in Schmorl Node in a Patient With Gastric Cancer

Rong Lin et al.

Summary: A 78-year-old man diagnosed with gastric adenocarcinoma underwent F-18-FDG and Ga-68-FAPI-04 PET/CT scans, which showed intense radioactivity in the gastric cancer and increased uptake in the benign Schmorl node. Two months after radical gastrectomy, a follow-up CT revealed no change in the Schmorl node in the T5 vertebrae.

Head-to-head intra-individual comparison of biodistribution and tumor uptake of 68Ga-FAPI and 18F-FDG PET/CT in cancer patients

Frederik L. Giesel et al.

Summary: The study aimed to compare Ga-68-FAPI and F-18-FDG in organ biodistribution and tumor uptake in PET/CT, showing that Ga-68-FAPI has lower uptake in most normal tissues compared to F-18-FDG, resulting in significantly higher TBRs at some sites for Ga-68-FAPI.

68Ga-FAPI-PET/CT in patients with various gynecological malignancies

Katharina Dendl et al.

Summary: Ga-68-FAPI-PET/CT demonstrates high tracer uptake for staging and follow-up of gynecological tumors, with sharp contrasts in primary and metastatic lesions and higher TBRs than F-18-FDG-PET/CT. The presence or absence of the menstrual cycle seems to correlate with FAPI accumulation in the normal endometrium and breast.
