Summary: Total body irradiation is an effective conditioning regimen for high risk or relapsed/refractory leukemia patients undergoing allogeneic stem cell transplantation. Pulmonary toxicity, particularly idiopathic pneumonia syndrome (IPS), remains a common adverse effect, with factors such as TBI regimen, dose rate, and pulmonary shielding influencing the risk. Further study is recommended to better understand and define IPS, as well as explore differences in risk factors between pediatric and adult patients.
