Microvascular Inflammation and Cardiovascular Prevention: The Role of Microcirculation as Earlier Determinant of Cardiovascular Risk

Alessandro Mengozzi et al.

Summary: Addressing inflammation and targeting microvascular phenotype are crucial in achieving satisfactory cardiovascular prevention, as factors like adipose tissue, mitochondrial function, hyperglycemic damage, and epigenetic signature play pivotal roles in promoting microvascular inflammation. Understanding the mechanisms and clinical potential of metabolic memory is essential to combat the detrimental role of inflammation in cardiometabolic diseases.

Targeting SIRT1 Rescues Age- and Obesity-Induced Microvascular Dysfunction in Ex Vivo Human Vessels

Alessandro Mengozzi et al.

Summary: This study aimed to investigate the role of SIRT1 in microvascular dysfunction caused by age and obesity in humans. The results showed that SIRT1 is a central regulator of early microvascular damage caused by age and obesity. Through the regulation of mitochondrial protein expression and mitochondrial reactive oxygen species levels, it influences nitric oxide availability and the expression of proteins involved in the mitochondria respiratory chain. Therapeutic modulation of SIRT1 can restore obesity- and age-related endothelial dysfunction.

Glomerular hyperfiltration in morbid obesity: Role of the inflammasome signalling

Diego Moriconi et al.

Summary: Obesity is associated with glomerular hyperfiltration, and inflammasome signaling may play a role in this process. This study found that weight loss can reduce hyperfiltration in most obese patients, but not all. Patients who did not show a decrease in IL-1 beta/Caspase-1 levels had persistent hyperfiltration. Early intervention targeting inflammasome signaling may be important in preventing the development of obesity-related kidney disease.

Summary: This study compared NO-related markers, CK levels, and uACR between young healthy black and white adults, finding black individuals to have a less favorable NO profile and higher levels of CK and MDA, suggesting a vulnerability to early hypertension development.

Microvascular Ageing Links Metabolic Disease to Age-Related Disorders: The Role of Oxidative Stress and Inflammation in Promoting Microvascular Dysfunction

Alessandro Mengozzi et al.

Summary: This review explores the connections between microvascular dysfunction, aging, and metabolic disorders, highlighting the major roles of inflammation and oxidative stress. Factors such as older age, hypertension, nutrient abundance, and hyperglycemia contribute to a persistent low-grade inflammatory response that impairs endothelial function and increases local inflammation.

Obesity, Adipose Tissue and Vascular Dysfunction

Mascha Koenen et al.

Summary: Cardiovascular diseases, as the leading cause of death worldwide, are strongly associated with overweight and obesity, which contribute to vascular dysfunction and cardiovascular diseases. The abnormal remodeling of specific adipose tissue depots during obesity plays a critical role in the development of hypertension, endothelial dysfunction, and vascular stiffness, with fat distribution being a key determinant of the risk for cardiovascular diseases.

Hypertension preserves the magnitude of microvascular flow-mediated dilation following transient elevation in intraluminal pressure

William E. Hughes et al.

Summary: The study found that exposure to transient increases in intraluminal pressure reduced flow-mediated dilation (FMD) in arterioles from non-CAD and CAD patients, while arterioles from non-CAD patients with hypertension did not show a reduction. This suggests a compensatory switch from NO to H2O2 as the mechanism of dilation in hypertension.
