Transthoracic esophagectomy compared to transhiatal extended gastrectomy for adenocarcinoma of the esophagogastric junction: a multicenter retrospective cohort study

Carlo Alberto De Pasqual et al.

Summary: This study compared the outcomes of transhiatal extended gastrectomy (TEG) and transthoracic esophagectomy (TTE) for Siewert type II tumors. The results showed that TEG was associated with a higher post-operative mortality rate, while TTE had a higher rate of complete tumor resection.

The abdominal-transhiatal surgical approach versus the thoracoabdominal surgical approach in Siewert type II adenocarcinoma of the esophagogastric junction: protocol for a multicenter prospective, open, parallel, and randomized controlled trial

Qiying Song et al.

Summary: This study aims to compare the surgical outcomes between the abdominal-transhiatal (TH) and right thoracoabdominal (RTA) approaches in patients with resectable Siewert type II adenocarcinoma of the esophagogastric junction (ST-II AEG). The study is a prospective randomized controlled trial, with the primary endpoint being the 3-year disease-free survival (DFS) following the operation. Secondary endpoints include the rate of R0-resection, the number and site of lymph nodes infiltrated and dissected, postoperative complications, hospital days and life quality. The findings of this study are expected to provide high-level clinical evidence for clinical decision-making on the appropriate surgical approach for patients with ST-II AEG.

Summary: This study compared the efficacy of TA and TT surgical approaches for Siewert type II AEG. The results showed that both approaches had comparable surgical and oncological outcomes. However, more research is needed to confirm this conclusion.

Transthoracic, thoracoabdominal, and transabdominal surgical approaches for gastric cardia adenocarcinomas: a survival evaluation based on a cohort of 7103 patients

Yao Chen et al.

Summary: This study compared the survival outcomes of different surgical approaches for gastric cardia adenocarcinoma (GCA) and found that the thoracoabdominal and transabdominal approaches were significant risk factors for poor survival. Patients treated with the transthoracic approach had the best survival rate.

Summary: A retrospective cohort study compared thoracic epidural analgesia (TEA) with paravertebral analgesia (PVA) in 25 consecutive thoracolaparoscopic Ivor Lewis esophagectomies, finding that TEA was superior to PVA in managing postoperative pain during the first three days, but both methods provided adequate analgesia with a median highest recorded Numeric Pain Rating Scale (NRS) of 4 or lower.

Esophagectomy or Total Gastrectomy for Siewert 2 Gastroesophageal Junction (GEJ) Adenocarcinoma? A Registry-Based Analysis

Sivesh K. Kamarajah et al.

Summary: This population-based cohort study compared survival outcomes between esophagectomy and total gastrectomy for the treatment of Siewert 2 GEJ adenocarcinoma. Despite similar lymph node harvest, length of stay, and 90-day mortality, esophagectomy was found to be prognostically superior to gastrectomy in this study.
