A Serum Biomarker Panel Can Accurately Identify Mucosal Ulcers in Patients With Crohn's Disease

Ariela K. Holmer et al.

Summary: This study investigated the predictive value of the Endoscopic Healing Index (EHI) in assessing endoscopic inflammation in Crohn's disease. The results showed that EHI is independently associated with ulcer size and can accurately identify large or very large ulcers. A cutoff of 50 points reliably indicates the presence of mucosal ulcers, while a cutoff of 20 points reliably rules out mucosal ulcers, providing guidance for treatment adjustment.

Summary: This post hoc analysis of clinical trial data showed that SES-CD and MM-SES-CD are comparable to the Rutgeerts score in predicting recurrence of postoperative Crohn's disease. Clinicians and trialists should consider using the SES-CD or MM-SES-CD to assess patients with postoperative CD.

Mild neoterminal ileal post-operative recurrence of Crohn's disease conveys higher risk for severe endoscopic disease progression than isolated anastomotic lesions

Salam P. Bachour et al.

Summary: There is conflicting data on the impact of isolated post-operative anastomotic inflammation on future disease progression in Crohn's disease patients. This study found that post-operative ileal disease recurrence, rather than isolated anastomotic inflammation, is associated with an increased risk of severe endoscopic disease progression.

The IBD-DCA and GHAS have the closest estimated predicted probability curve to the ideal curve, suggesting better predictability for recurrence.

Summary: This study retrospectively examined late recurrence in CD patients after ileocecal resection, finding that 40.7% of patients experienced late post-operative recurrence after a median follow-up of 3.5 years. Summary: This study investigated the rates of clinical and surgical recurrence of Crohn's disease after surgery, and found that therapy modification based on post-operative endoscopic findings can impact disease recurrence rates. However, while initiating immunosuppressant or biological therapy after endoscopy showed a modest decrease in risk of clinical recurrence for patients with Rutgeerts scores of i3 or i4, this effect was not observed for patients with scores of i2.
