Human in silico trials for parametric computational fluid dynamics investigation of cerebrospinal fluid drug delivery: impact of injection location, injection protocol, and physiology

Mohammadreza Khani et al.

Summary: This study used computational fluid dynamics and in vitro model verification to assess intrathecal drug delivery in an anatomically idealized model of the human CSF system. The results showed that injection location and volume can significantly affect the distribution of drugs in the brain. The movement of CSF induced by cardiac and respiratory factors also plays an important role in drug distribution.

Systemic hypertonic saline enhances glymphatic spinal cord delivery of lumbar intrathecal morphine

Kim J. Blomqvist et al.

Summary: The blood-brain barrier limits drug delivery to the central nervous system, but the glymphatic system provides a route for drugs to reach deep brain structures. This study found that hyperosmotic interventions can enhance the delivery of intrathecally administered morphine to the spinal cord, improving its efficacy.

The glymphatic system: implications for drugs for central nervous system diseases

Terhi J. Lohela et al.

Summary: Evidence for a fluid clearance pathway called the glymphatic system in the central nervous system has increased in the past decade. The glymphatic system clears metabolic waste from the brain interstitium by directing the flow of cerebrospinal fluid. It can be modulated by drugs such as anaesthetics and non-pharmacological factors like sleep, and its dysfunction is associated with central nervous system disorders like Alzheimer's disease. While primarily studied in rodents, multiple neuroimaging modalities suggest the existence of a similar transport system in the human brain. This review provides an overview of the evidence for the glymphatic system, its role in disease, and opportunities for therapeutic intervention.

SPECT/CT imaging reveals CNS-wide modulation of glymphatic cerebrospinal fluid flow by systemic hypertonic saline

Tuomas O. Lilius et al.

Summary: The study showed that systemic hypertonic saline (HTS) increases the intracranial exposure of cerebrospinal fluid-administered tracer in the brain, but has minimal effects on brain clearance. SPECT/CT imaging is a valuable approach for studying glymphatic drug delivery.

Cerebrospinal fluid outflow: a review of the historical and contemporary evidence for arachnoid villi, perineural routes, and dural lymphatics

Steven T. Proulx

Summary: Cerebrospinal fluid is produced in the ventricles of the brain and circulates through the subarachnoid space of the skull and spinal column to provide buoyancy and maintain fluid homeostasis. Recent studies challenge the traditional belief that CSF drains through arachnoid villi, suggesting it may instead flow through lymphatic vessels.

Strategies for delivering therapeutics across the blood-brain barrier

Georg C. Terstappen et al.

Summary: The blood-brain barrier presents a challenge for delivering therapeutics to the central nervous system, particularly for biopharmaceuticals. Non-invasive approaches have been developed to help overcome this challenge, with increasing research efforts focusing on technologies to deliver therapeutics to the CNS, some of which have entered clinical testing. Developing effective strategies for crossing the blood-brain barrier is crucial for treating CNS disorders, with a particular emphasis on non-invasive methods such as receptor-mediated transcytosis and the use of neurotropic viruses, nanoparticles, and exosomes.
