Development of a Generic Physiologically-Based Pharmacokinetic Model for Lactation and Prediction of Maternal and Infant Exposure to Ondansetron via Breast Milk

Kathleen M. Job et al.

Summary: A physiologically-based pharmacokinetic lactation model for ondansetron was developed and validated, showing good agreement with observed data. The model predicted low relative infant doses, indicating that mothers receiving ondansetron treatment can safely breastfeed. The open-source model can be adapted to study other drugs and populations.

Summary: In this study, an IVIVE model was developed to predict the human milk/plasma drug concentration ratios, showing superior prediction accuracy compared to previous models. The model performed well in predicting passive diffusion drugs but had lower accuracy for P-gp substrates.

Technical Report: Breastfeeding and the Use of Human Milk

Joan Younger Meek et al.

Summary: Breastfeeding and human milk are recommended standards for infant feeding and have many medical and neurodevelopmental benefits. The American Academy of Pediatrics and the World Health Organization recommend exclusive breastfeeding for approximately 6 months and continued breastfeeding, along with complementary foods, for at least 2 years. Medical contraindications to breastfeeding are rare. Birth hospitals should implement maternity care practices that improve breastfeeding initiation, duration, and exclusivity. Pediatricians play a crucial role in advocating for breastfeeding and need to be trained in the benefits and management of breastfeeding. Efforts to improve breastfeeding rates must address existing disparities and the impact of racism.

Summary: This study aimed to predict potential risks to neonates from maternal drug intake during lactation by developing a lactation PBPK model that considers physiological properties, drug ionization, and distribution, along with calculating infant doses.
