An Educational Need Regarding Treatment-Related Infertility and Fertility Preservation: a National Survey Among Members of the Dutch Society for Medical Oncologists

E. M. Krouwel et al.

Summary: Cancer diagnosis and treatment can affect fertility and reproductive planning in patients, but the education and practice of medical oncologists in regards to fertility preservation is still uncertain. A survey of Dutch Society for Medical Oncology members found that the majority of oncologists believe it is their responsibility to discuss the impact of cancer treatment on fertility, but many do not often or always have these discussions. There are barriers and variations in knowledge and practice among oncologists when it comes to fertility preservation.

Understanding the barriers to, and facilitators of, ovarian toxicity assessment in breast cancer clinical trials

Wanyuan Cui et al.

Summary: This study aimed to determine the barriers to and facilitators of ovarian toxicity assessment in breast cancer clinical trials. Through semi-structured interviews with stakeholders from multiple countries, the main barrier identified was the lack of consideration. However, most participants believed that evaluating the impact of breast cancer treatments on ovarian function is valuable. Suggested strategies to increase ovarian toxicity assessment included including it in clinical trial design guidelines and stakeholder advocacy.

The effect of medication on serum anti-mullerian hormone (AMH) levels in women of reproductive age: a meta-analysis

Wei-Wei Yin et al.

Summary: Serum AMH levels may fluctuate in the short term after medication application. Different medications have different effects on AMH. Oral contraceptives, metformin, and clomiphene citrate lead to a decrease in AMH levels, while dehydroepiandrosterone and vitamin D lead to an increase. Gonadotropin-releasing hormone agonist causes dynamic changes. The impacts of these medications should be considered when using AMH as a marker of ovarian reserve.

Summary: Despite a wealth of data on immune-related adverse events, the impact of immune checkpoint inhibitors on gonadal function remains understudied. Evidence suggests that ICIs may pose risks to fertility through hypogonadism, emphasizing the need for further investigation due to the increasing use of ICIs in various populations.

Difference in Estimation of Side Effects of Chemotherapy between Physicians and Patients with Early-Stage Breast Cancer: The Use of Patient Reported Outcomes (PROs) in the Evaluation of Toxicity in Everyday Clinical Practice

Mirjana Pavlovic Mavic et al.

Summary: The study showed a discrepancy between patient and physician perceptions of treatment side effects, with physicians reporting side effects much less frequently than patients. Therefore, the use of patient-reported outcomes (PROs) in clinical practice can help avoid subjective estimations by physicians, improve patient quality of life, and ultimately lead to better treatment outcomes.

Summary: The study shows that TKIs have a negative impact on oocyte and sperm maturation, gonadal function, and overall fertility potential, with a lower risk of teratogenicity to a fathered pregnancy with TKI use. The complexity of pregnancy outcomes and results in women taking TKIs compared to men is higher, and it is advised for women to avoid conception while taking TKIs.

Molecular Mechanism and Prevention Strategy of Chemotherapy- and Radiotherapy-Induced Ovarian Damage

Seongmin Kim et al.

Summary: Fertility preservation is an emerging discipline with substantial clinical value for young cancer patients. Chemotherapy and radiation can harm ovarian function, with oxidative stress and oocyte radiosensitivity being identified as main causes of ovarian damage. The importance of oncofertility strategies that consider various factors for high-quality care for young female cancer patients is highlighted in this review.
