Acute neurofunctional effects of escitalopram during emotional processing in pediatric anxiety: a double-blind, placebo-controlled trial

Lu Lu et al.

Summary: The study found that escitalopram can improve emotional processing speed in adolescents with generalized anxiety disorder, with a positive impact on the connectivity of the amygdala to bilateral ventromedial prefrontal cortex and right angular gyrus. Baseline amygdala-vmPFC connectivity and escitalopram-induced increased amygdala-angular gyrus connectivity at week 2 can predict the magnitude of subsequent improvement in anxiety symptoms.

Research Review: Pediatric anxiety disorders - what have we learnt in the last 10 years?

Jeffrey R. Strawn et al.

Summary: Anxiety disorders are common, early-onset conditions with various risk factors and are effectively treated with cognitive behavioral therapy, selective serotonin reuptake inhibitors, and serotonin-norepinephrine reuptake inhibitors. The disorders are associated with distinct neurobiological features and show consistent response to treatment, although questions remain about risk factors and neurobiology's impact on treatment outcomes.

Summary: In adolescents with generalized anxiety disorder (GAD), escitalopram increased amygdala-prefrontal connectivity within the first 2 weeks of treatment, and the magnitude of this early change predicted subsequent clinical improvement.

Pediatric Therapeutic Drug Monitoring for Selective Serotonin Reuptake Inhibitors

Jeffrey R. Strawn et al.

Summary: Therapeutic drug monitoring (TDM) is not commonly used in child and adolescent psychiatry, but it can help understand individual variability in antidepressant pharmacokinetics, drug interactions, adherence, and genetic and metabolic gene variations. TDM in SSRI-treated youths is evolving, but faces barriers in pediatric patients.
