Intravenous Thrombolysis in Patients With Ischemic Stroke and Recent Ingestion of Direct Oral Anticoagulants

Thomas R. Meinel et al.

Summary: This study aimed to determine the risk of symptomatic intracranial hemorrhage (sICH) associated with the use of intravenous thrombolysis (IVT) in patients with recent intake of direct oral anticoagulants (DOACs) after ischemic stroke. The findings suggest that there is no excessive harm associated with off-label IVT in selected patients after ischemic stroke with recent DOAC ingestion.

Association of Recent Use of Non-Vitamin K Antagonist Oral Anticoagulants With Intracranial Hemorrhage Among Patients With Acute Ischemic Stroke Treated With Alteplase

Wayneho Kam et al.

Summary: This study aims to evaluate the safety and functional outcomes of intravenous alteplase in patients with acute ischemic stroke who were taking non-vitamin K antagonist oral anticoagulants (NOACs) prior to stroke, and compare the outcomes with patients who were not taking long-term anticoagulants.

Association Between Prior Anticoagulation and Thrombus Composition in Mechanical Thrombectomy Patients with Atrial Fibrillation

Chendong Wang et al.

Summary: This study aimed to investigate the association of prior anticoagulation treatment with thrombus composition and clinical outcome in atrial fibrillation (AF) patients with acute ischemic stroke (AIS). The study found that thrombi retrieved from AF patients with prior anticoagulation treatment contained more fibrins, more platelets, and fewer red blood cells compared to those without anticoagulation treatment. However, no significant differences were observed in terms of successful recanalization and functional independence.

Summary: The study analyzed data from the German Stroke Registry-Endovascular Treatment and found that endovascular treatment can be safely and successfully performed in orally anticoagulated stroke patients, although the frequency of good outcomes at 3 months was lower in these patients.

Recanalisation therapies for acute ischaemic stroke in patients on direct oral anticoagulants

David J. Seiffge et al.

Summary: This review discusses the primary therapeutic option for stroke prevention in atrial fibrillation patients, Direct Oral Anticoagulants (DOACs); it evaluates different approaches to patient selection, including time since last DOAC intake, drug-specific coagulation assays, and the use of reversal agents; precision medicine using a tailored approach seems to be a reasonable choice based on clinicoradiological information, anticoagulant activity, and the use of specific reversal agents only if necessary.
