Point mutations in the squalene epoxidase erg1 and sterol 14-α demethylase erg11 gene of T indotineae isolates indicate that the resistant mutant strains evolved independently

Anke Burmester et al.

Summary: The T indotineae population exhibits a high level of terbinafine and azole resistance due to various mutations in the erg1 and Erg11 genes. It is crucial to understand the molecular mechanisms underlying azole resistance and the need to analyze genes involved in resistance mechanisms to improve fungal treatment strategies.

Gene Amplification of CYP518: a New Mechanism of Resistance to Azole Compounds in Trichophyton indotineae

Tsuyoshi Yamada et al.

Summary: Trichophyton indotineae strains resistant to itraconazole and voriconazole were found to be associated with overexpression of TinCYP518 due to additional copies of this gene. RNA interference of TinCYP518 restored drug susceptibility in resistant strains, while overexpression of TinCYP518 conferred resistance in susceptible strains of T. indotineae.

Multidrug resistant tinea corporis/cruris: Response to voriconazole

Ananta Khurana et al.

Summary: Due to increasing drug resistance and erratic clinical responses, treatment options for refractory dermatophytosis are limited. However, refractory dermatophytosis has become a significant public health issue in India in recent years. This article presents a patient who failed four systemic antifungals sequentially but was cured with a 2-week course of voriconazole, an antifungal not commonly used for dermatophytosis.

Effect of Different Itraconazole Dosing Regimens on Cure Rates, Treatment Duration, Safety, and Relapse Rates in Adult Patients With Tinea Corporis/Cruris A Randomized Clinical Trial

Ananta Khurana et al.

Summary: This randomized clinical trial compares the efficacy and safety of different doses of itraconazole for the treatment of tinea corporis/cruris. The study found that the 400 mg dose was superior to the 100 mg and 200 mg doses in terms of cure rates and treatment duration. However, higher doses of treatment incurred higher costs.

An exploratory pilot analysis of the optimal pellet number in 100 mg of itraconazole capsule to maximize the surface area to satisfy the Noyes-Whitney equation

Kabir Sardana et al.

Summary: The rising prevalence of recalcitrant dermatophytoses has led to increased use of the antifungal itraconazole as a potential alternative to terbinafine. Due to varying bioavailability and quality among different brands, it is important for clinicians to differentiate between good and poorly manufactured brands for optimal clinical outcomes. A pellet count of 560 or above provides an optimal surface area for absorption of itraconazole from the gastrointestinal tract, serving as a potential cut off to distinguish poor quality brands in the absence of bioequivalence studies with the innovator brand.

Complementary effect of mechanism of multidrug resistance in Trichophyton mentagrophytes isolated from human dermatophytoses of animal origin

Sebastian Gnat et al.

Summary: This study analyzed clinical isolates of Trichophyton mentagrophytes to investigate their resistance mechanisms to antifungal drugs. The research found differences in gene expression and sequences, highlighting the role of the ABC transporter family and potential use of milbemycin oxime in treating refractory dermatomycoses with azole drugs.

Itraconazole resistance of Trichophyton rubrum mediated by the ABC transporter TruMDR2

Tsuyoshi Yamada et al.

Summary: The terbinafine resistant strain of Trichophyton rubrum TIMM20092 exhibited reduced sensitivity to itraconazole (ITC) and voriconazole (VRC), with the up-regulation of multidrug transporters TruMDR2 and TruMDR3 playing a key role in its resistance mechanisms. Deletion of TruMDR3 in TIMM20092 eliminated resistance to VRC and significantly reduced resistance to ITC, highlighting TruMDR2 as a major contributor to ITC resistance in this strain.

Checkerboard Analysis To Evaluate Synergistic Combinations of Existing Antifungal Drugs and Propylene Glycol Monocaprylate in Isolates from Recalcitrant Tinea Corporis and Cruris Patients Harboring Squalene Epoxidase Gene Mutation

Kabir Sardana et al.

Summary: This study aimed to test the synergistic combinations of oral and topical antifungals for treating recalcitrant tinea corporis/cruris. It was found that many isolates showed high MICs of terbinafine, but synergistic interactions were noted with combinations of itraconazole with luliconazole and other drugs.

Predicting a therapeutic cut-off serum level of itraconazole in recalcitrant tinea corporis and cruris-A prospective trial

Ananta Khurana et al.

Summary: With increasing resistance to terbinafine, fluconazole, and griseofulvin, itraconazole is now commonly used as a first-line treatment for tinea corporis/cruris. However, therapeutic failures may occur with inadequate serum levels of itraconazole, while higher levels are associated with positive treatment outcomes. In India, itraconazole has shown low MICs against the prevalent strain, making therapeutic failures uncommon.

Immunopathogenesis of Dermatophytoses and Factors Leading to Recalcitrant Infections

Kabir Sardana et al.

Summary: The pathogenesis of dermatophytic infections involves the interaction of three main factors: the fungus, host defense, and immune response. Immune response depends on pathogen-associated molecular patterns and recognition receptors, with zoophilic species being eliminated due to a brisk immune response. In contrast, anthropophilic species subvert the immune response via mannans to evade elimination.

Taxonomy of the Trichophyton mentagrophytes/T. interdigitale Species Complex Harboring the Highly Virulent, Multiresistant Genotype T. indotineae

Chao Tang et al.

Summary: In India, a severe outbreak of highly virulent and multi-resistant dermatophytosis by species in the Trichophyton mentagrophytes/T. interdigitale complex is ongoing, and the accurate identification of the etiologic agent is a debated issue. Molecular, morphological, and physiological characteristics were used in a taxonomic study to define species limits. The study found that while T. mentagrophytes s. str. and T. interdigitale s. str. are difficult to distinguish, they are different from the Indian genotype T. indotineae in the HMG gene sequence. These characteristics may be driving separate evolutionary trends.
