Pathogenesis of chronic heart failure: cardiovascular aging, risk factors, comorbidities, and disease modifiers

Filippos Triposkiadis et al.

Summary: Chronic heart failure (HF) is rare in the young and common in the elderly. The causes of HF in the young are usually specific, such as adult congenital heart disease, cardiomyopathies, myocarditis, or cardiotoxicity. On the other hand, the mechanisms underlying HF development in the elderly have not been fully understood. It is proposed that in most elderly patients, HF is the consequence of cardiovascular aging accelerated by specific risk factors, such as hypertension, obesity, type 2 diabetes mellitus (T2DM), coronary artery disease (CAD), and valvular heart disease (VHD), which affect both the heart and the vasculature.

Summary: The study found that more than half of heart failure patients had reduced muscle mass, and lower values of both muscle and fat mass were associated with higher mortality in heart failure.

The ergoreflex: how the skeletal muscle modulates ventilation and cardiovascular function in health and disease

Alberto Aimo et al.

Summary: The ergoreflex, activated by physical activity, plays a role in regulating ventilation and cardiovascular function. Its two components, the mechanoreflex and metaboreflex, respond to muscle contraction and metabolite accumulation in exercising muscles. Patients with heart failure and neuromuscular disorders often experience increased ergoreflex sensitivity, leading to symptoms like dyspnoea and reduced tolerance to physical exercise. Exercise training can help mitigate ergoreflex sensitivity, restore balance, and improve exercise tolerance in these patients.

Frailty, sarcopenia and cachexia in heart failure patients: Different clinical entities of the same painting

Matteo Beltrami et al.

Summary: Heart failure in elderly patients is a systemic syndrome where advanced age, comorbidities, frailty, and impaired cognition significantly impact outcomes. Cardiac cachexia, sarcopenia, and frailty often coexist in these patients and need to be addressed to improve quality of life and reduce rehospitalization rates. Treatment strategies focus on increasing muscle mass and strength, along with utilizing common HF drugs and physical exercise programs.
