VFDB 2022: a general classification scheme for bacterial virulence factors

Prevalence of insertion sequence elements in plasmids relating to mgrB gene disruption causing colistin resistance in Klebsiella pneumoniae

Summary: Colistin is a last resort antibiotic for treating carbapenemase producing Klebsiella pneumoniae. The insertion of IS elements into the mgrB gene is a mechanism that causes colistin resistance. Plasmids carrying IS elements are common in K. pneumoniae, with ISKpn25 being mostly carried by IncFIB(pQil) plasmids, while ISKpn26 and ISKpn14 are often associated with IncFII(pHN7A8) plasmids. ISKpn25 is identified in 26 out of 34 countries that contain any of the IS elements, while ISKpn26, ISKpn14, and IS903B are most frequently found in China. Plasmids carrying ISKpn25, ISKpn14, and ISKpn26 have a higher count of carbapenemase genes compared to IS903B-carrying plasmids. IS903B-bearing plasmids have a higher count of environmental source isolations compared to ISKpn25, ISKpn14, and ISKpn26 bearing plasmids. The presence of carbapenemase in combination with IS elements could lead to an extensively drug-resistant profile in K. pneumoniae, limiting treatment options.

Molecular epidemiology of carbapenem-resistant hypervirulent Klebsiella pneumoniae in China

Summary: The epidemiological features and potential threat to human health of the newly emerged carbapenem-resistant hypervirulent Klebsiella pneumoniae (CR-HvKP) are currently unknown. This study analyzed 784 CRKP strains collected from three hospitals in China and found that the proportion of CRKP strains among clinical K. pneumoniae strains increased sharply. A significant number of these CRKP strains carried a virulence-encoding plasmid, but only a small percentage exhibited a hypervirulent phenotype. Genetic markers were not strongly correlated with hypervirulent phenotypes, indicating that additional factors may contribute to the hypervirulence of CRKP. The transmission of these CRKP strains in China likely involved multiple clones of ST11 and the major mechanism of carbapenem resistance was the carriage of IncFII pSWU01-like, bla (KPC-2)-bearing plasmids. These findings highlight the rapid increase in prevalence of CRKP strains carrying virulence plasmids in China and the need for better definition and screening of truly hypervirulent CR-HvKP strains in clinical settings.

Summary: The global prevalence of colistin-resistant Klebsiella pneumoniae (ColRkp) with chromosomal and plasmid-mediated LPS alterations has increased steadily. Our study demonstrated the extensively and pandrug-resistant characteristics of ColRkp among carbapenem-resistant K. pneumoniae (CRkp). We discovered significant associations between extensively drug-resistant bacteria (XDR) and pandrug-resistant bacteria (PDR) ColRkp.

Transmission of Nonconjugative Virulence or Resistance Plasmids Mediated by a Self-Transferable IncN3 Plasmid from Carbapenem-Resistant Klebsiella pneumoniae

Summary: Klebsiella pneumoniae poses a critical challenge to clinical and public health due to the global spread of conjugative and nonconjugative plasmids associated with carbapenem-resistant, hypervirulent, and even dual-resistant and hypervirulent strains. This study identified a clinically significant CRKP strain and demonstrated the high conjugation ability of its IncN3 plasmid, which can mobilize nonconjugative virulence or resistance plasmids. The findings suggest a need for further monitoring of the transmission of pathogenicity and genetic evolution of carbapenem-resistant and hypervirulent K. pneumoniae strains.

Insertion sequence mediating mrgB disruption is the major mechanism of polymyxin resistance in carbapenem-resistant Klebsiella pneumoniae isolates from China

Summary: This study evaluated the incidence of polymyxin resistance in clinical CRKP isolates in China and characterized the molecular mechanisms underlying polymyxin-resistant CRKP isolates. The results showed a 2.2% polymyxin resistance rate in a multicentre cohort, with key genetic variations involving disruptions in mgrB and mutations in pmrA, pmrB, and phoP identified in PR-CRKP isolates. These findings highlight the multifaceted molecular mechanisms of polymyxin resistance in CRKP.

Hypervirulent Klebsiella pneumoniae Sequence Type 420 with a Chromosomally Inserted Virulence Plasmid
