High prevalence of colistin resistance and mcr-9/10 genes in Enterobacter spp. in a tertiary hospital over a decade

Weichao Liao et al.

Summary: This study investigated the prevalence of colistin resistance and mcr genes in clinical Enterobacter spp. isolates in a tertiary hospital in China between 2011 and 2020. The results showed a high prevalence of colistin resistance, with a range of 17.1% to 34.5% and an overall prevalence of 22.2%. No mcr-1 to mcr-8 genes were identified in the colistin-resistant Enterobacter spp. isolates, but mcr-9 and mcr-10 were detected at rates of 8.4% and 12.6%, respectively. Most mcr-9/10 genes were located on plasmids, and the plasmid backbones differed between mcr-9 and mcr-10. This study highlights the urgent need to control the prevalence of colistin resistance and prevent the dissemination of mcr-9/10 genes in clinical Enterobacter spp. isolates.

Characterization of blaNDM-positive Enterobacteriaceae reveals the clonal dissemination of Enterobacter hormaechei coharboring blaNDM and tet(X4) along the pork production chain

Ruichao Li et al.

Summary: This study elucidated the epidemiology and transmission characteristics of bla(NDM)-positive Enterobacteriaceae strains in two links of the pork production chain. Coexistence of bla(NDM) and tet(X4) was found in isolates from a pig slaughterhouse and a retail pork market, with evidence of clonal spread from slaughterhouse to market. The slaughterhouse also served as a possible reservoir for bla(NDM)-positive E. coli strains. The IncX3 plasmid was identified as the main plasmid carrying bla(NDM), with conjugative transmission observed in most strains. Nonconjugative plasmids carrying tet(X4) had identical architectures across different species. Phylogenetic analysis suggested translocation of tet(X4)-positive circular intermediates and recombination events in bla(NDM)-bearing IncX3 plasmids in the formation of certain strains.

Frequent convergence of mcr-9 and carbapenemase genes in Enterobacter cloacae complex driven by epidemic plasmids and host incompatibility

Tingting Xu et al.

Summary: The convergence of MCR and carbapenemase genes in Enterobacter cloacae complex (ECC) is widespread and increasing. The study found a high prevalence of MCR-ECC and MCR-CREC worldwide, indicating frequent co-occurrence of mcr-9/10 and carbapenemase genes in ECC. Furthermore, epidemic plasmids were identified in ECC that contributed to the dissemination of mcr-9 globally.

Summary: An outbreak of KPC-3-producing Enterobacterales infection occurred in China, with the involvement of clonal expansion and transmission across different species. These KPC-3 strains exhibited high virulence and readily developed resistance. Close monitoring of the spread of bla(KPC-3)-harboring plasmids and these strains is necessary in China and globally.

Characterization of a Plasmid-Encoded Resistance-Nodulation Division Efflux Pump in Klebsiella pneumoniae and Klebsiella quasipneumoniae from Patients in China

Ruowen He et al.

Summary: The study identified two MDR Klebsiella pneumoniae isolates carrying mcr-1 from patients with urinary tract infections, and one MDR Klebsiella quasipneumoniae isolate carrying tmexCD1-toprJ1 from a patient with bloodstream infection. The addition of an efflux pump inhibitor significantly reduced tigecycline resistance in these isolates. Additionally, a plasmid carrying tmexCD1-toprJ1 was successfully transferred via conjugation from K. quasipneumoniae to Escherichia coli J53 for further analysis.

A Novel Transferable Resistance-Nodulation-Division Pump Gene Cluster, tmexCD2-toprJ2, Confers Tigecycline Resistance in Raoultella ornithinolytica

Cheng-Zhen Wang et al.

Summary: The study identified a novel mobile gene cluster, tmexCD2-toprJ2, present in Raoultella ornithinolytica, which confers resistance to multiple antimicrobials and may have originated from a Pseudomonas species. The expression of tmexCD2-toprJ2 in Escherichia coli resulted in increased resistance to tigecycline and decreased susceptibility to other antimicrobials. Genetic context analyses demonstrated that tmexCD2-toprJ2 might have been captured and mobilized by a XerD-like tyrosine recombinase system.

Summary: This study detected and characterized the mcr-9 gene in a colistin-susceptible Enterobacter hormaechei isolate from a pediatric patient in Lebanon. The gene was found to be chromosomal and related to the QseCB regulatory system. The silent spread of mcr-9 in the globally successful ST-78 Enterobacter lineage is concerning and requires close monitoring.

Characterization of TMexCD3-TOprJ3, an RND-Type Efflux System Conferring Resistance to Tigecycline in Proteus mirabilis, and Its Associated Integrative Conjugative Element

Qian Wang et al.

Summary: The emergence and transmission of novel antimicrobial resistance genes, such as the plasmid-encoded RND efflux pump TMexCD1-TOprJ1, pose a great threat to global public health by reducing the sensitivity of multiple antimicrobials. This study identified a pandrug-resistant Proteus mirabilis isolate harboring a novel tmexCD3-toprJ3 gene cluster located on SXT/R391 ICE, expanding current knowledge on the transfer mechanisms of tmexCD1-toprJ1-like gene clusters among P. mirabilis isolates. Further genomic epidemiology investigations are warranted.

Institutional outbreak involving multiple clades of IMP-producing Enterobacter cloacae complex sequence type 78 at a cancer center in Tokyo, Japan

Sohei Harada et al.

Summary: Limited information has been available on the clinical and microbiological characteristics of IMP-producing Enterobacterales. Here, we report an institutional outbreak of IMP-producing Enterobacter cloacae complex (ECC) involving multiple clades of ECC sequence type (ST) 78 strains. Whole-genome sequencing analysis revealed three different clades of ST78 strains were responsible for the outbreak, with most IMP-producing ECC strains being susceptible to non-beta-lactam antibiotics and having relatively low minimum inhibitory concentrations to carbapenems, resulting in favorable treatment outcomes for patients compared to infections caused by other carbapenemase-producing Enterobacterales.

Plasmids Shape the Current Prevalence of tmexCD1-toprJ1 among Klebsiella pneumoniae in Food Production Chains

Summary: The study investigated the prevalence of tmexCD1-toprJ1-positive strains along the pork production chain, identifying a high distribution in IncFIB(Mar)/HI1B plasmids of K. pneumoniae. Furthermore, the potential transmission risk of such plasmids along the pork processing chain was observed, posing a threat to humans. The mobilization and dissemination of tmexCD1-toprJ1, especially when driven by plasmids, warrant sustained attention and investigations.
