Management of hypertensive emergencies and urgencies: narrative review

Hannah Jolly et al.

Summary: Hypertensive emergencies and hypertensive urgencies are distinguished by the presence of target organ damage. Understanding cerebral autoregulation is crucial for treatment decisions. Intravenous antihypertensive medication is required for hypertensive emergencies, while there is no clear evidence for the treatment of hypertensive urgencies.

Profile and management of hypertensive urgencies and emergencies in the emergency cardiology department of a tertiary hospital: a 12-month registry

Christos Fragoulis et al.

Summary: This study provides epidemiological data on the prevalence, clinical characteristics, and management of hypertensive urgencies and emergencies in an emergency department. Stress, increased salt intake, and medication non-adherence were identified as main triggers. Patients with urgencies commonly presented with dizziness and headache, while those with emergencies often showed symptoms of dyspnea and chest pain.

Modern Management of Hypertensive Emergencies

Gian Paolo Rossi et al.

Summary: Acute increases in blood pressure leading patients to emergency departments are common and represent a clinical challenge. Recent recommendations from the European Society of Cardiology suggest focusing on 'hypertensive emergencies' and using the BARKH strategy for identification. Improved understanding of the underlying pathophysiology has influenced the management of patients with hypertensive emergencies in recent years.

Hypertensive urgencies during the first wave of the COVID-19 pandemic in a tertiary hospital setting: a U-shaped alarming curve

Ioannis Leontsinis et al.

Summary: This study investigated the impact of the COVID-19 pandemic on emergency visits and admissions for uncontrolled hypertension. The results showed a decline in visits for hypertension urgencies during the rapid spread of the virus, which reversed after the decline in transmission rate. The study also found that the number of admissions for uncontrolled hypertension was related to the incidence of COVID-19 cases. The initial increase in admissions for uncontrolled hypertension may indicate blood pressure deregulation among the studied population.

Summary: This study investigated cardiac and vascular hypertension mediated organ damage (HMOD) in hypertensive urgencies (HU) and asymptomatic outpatients with grade 1-3 hypertension. The results showed that patients with HU had a better cardiac HMOD profile than outpatients with grade 3 hypertension.

Hypertensive emergencies in Asia: A brief review

Praew Kotruchin et al.

Summary: Hypertensive emergencies have varying prevalence rates across Asia, with stroke being the most common complication. Despite high hospitalization rates, the mortality rate remains low. The lack of data emphasizes the need for a multinational data repository and guidelines for managing hypertensive emergencies in Asian populations.

Impact of the COVID-19 pandemic on emergency department attendances and acute medical admissions

Michael E. Reschen et al.

Summary: The study analyzed the impact of the COVID-19 pandemic on hospital healthcare by comparing patient records from the emergency department and acute medicine department of a major UK hospital during the pandemic and the previous year. During the first wave, there was a significant decrease in ED attendance and medical admissions, with a subsequent return to normal levels within a year. COVID-19 admissions were more common in minority ethnic groups, while non-COVID-19 admissions had higher rates of hypertension, cerebrovascular disease, liver disease, and obesity.

Summary: The study suggests that pharmacological blood pressure intervention in the emergency department under universal health care can significantly reduce the risk of hospital revisits within 30 or 60 days for hypertensive crisis patients, especially among those on polypharmacy, but does not show a significant impact on incident stroke and cardiovascular mortality.

Clinical Characteristics and Predictors of All-Cause Mortality in Patients with Hypertensive Urgency at an Emergency Department

Jeong-Hun Shin et al.

Summary: This study investigated the characteristics and predictors of all-cause mortality in Korean emergency patients with hypertensive urgency, finding a 3-year all-cause mortality rate of 12.1% along with higher rates of emergency department revisits and readmission among non-survivors. Age over 60, male sex, history of chronic kidney disease, and proteinuria were identified as independent predictors of 3-year all-cause mortality.
